Le conseiller résidentiel, en collaboration avec les patients, les familles (telles que définies par le patient) et le personnel de toutes les disciplines et départements, est responsable de l'évaluation, de la planification, de la direction et de l'administration d'un soutien de qualité aux personnes vivant avec les effets d'une lésion cérébrale et d'une supervision étroite et intensive dans un cadre résidentiel. Le conseiller en résidence travaille en partenariat avec le patient et sa famille et, compte tenu de l’environnement du patient, est également responsable d’aider à la coordination des services en résidence et de fournir un traitement et des soins professionnels aux résidents. De plus, ce poste est chargé de fournir une gamme de services de conseil en résidence et de formation professionnelle aux clients et à leurs familles, de suivre et de rendre compte des progrès du traitement des résidents et d'assurer le leadership des pairs.
Ce service peut également inclure les composantes de base du service de formation à l'autonomie et des programmes de gestion comportementale spécifiques à la SR développés et/ou supervisés par le service professionnel financé par les Services de psychologie. À tout moment, le conseiller résidentiel soutient activement la philosophie, les buts et les objectifs du programme de soins centrés sur le patient et la famille (PFCC), les activités éducatives et programmatiques qui promeuvent et mettent en œuvre les comportements PFCC, et la coordination des opportunités permettant aux patients et aux familles de contribuer à la mission, la vision et les valeurs de VCBIS.
Diplôme ou baccalauréat en travail social, en psychologie, en counseling ou dans les domaines liés aux services à la personne.
Expérience antérieure de travail avec des personnes vivant avec les effets d’une lésion cérébrale un atout ou au moins deux ans d’expérience de travail dans la prestation de services de santé mentale ou de services sociaux.
Premiers soins et RCR C
Fournit un environnement stimulant ainsi qu'une surveillance et une sécurité étroites et intensives aux personnes vivant avec les conséquences d'une lésion cérébrale dans le programme résidentiel.
Enseigne des compétences de vie saine et organise des activités récréatives et sociales améliorant l'acquisition de compétences et le plaisir
Assure la présence aux rendez-vous médicaux, dentaires, de conseil ou autres services sociaux, y compris le transport si nécessaire
Utilise les techniques de gestion du comportement de manière appropriée avec les résidents
Facilite les séances de counseling de groupe et de formation professionnelle pour aider les résidents à développer leurs compétences dans des domaines cibles spécifiques de leur plan de soutien individuel.
Signale tous les risques ou préoccupations en matière de santé et de sécurité ainsi que les exigences d'entretien du site au superviseur.
Maintient une connaissance pratique des techniques sécuritaires de mécanique de carrosserie et utilise ces connaissances dans l’exécution de toutes les tâches.
Soumet les rapports d'exercices d'urgence, effectue des exercices d'évacuation d'urgence et complète la liste de contrôle de préparation aux situations d'urgence selon les directives du superviseur.
Rédiger des notes de progression conformément aux normes des politiques et procédures pour toutes les formations individuelles et collectives
Remplir la communication quotidienne sur le quart de travail et la documentation des données à la fin de chaque quart de travail
Capacité à assurer une surveillance visuelle et auditive des résidents
