Community Occupational Therapist – Community Advantage Rehabilitation (CAR) – a division of Lifemark. We are seeking Occupational Therapists in the following areas of Durham Region (Pickering, Ajax, Whitby, Oshawa, Bowmanville, Uxbridge, Newcastle)

As Durham Region’s largest community based rehabilitative therapy provider we have been providing community rehabilitation services for adults and children since 1997. Our Vision is to be recognized as a trusted and quality provider of healthcare services, helping adults and children to maximize their functional abilities within their environment and enable them to participate more fully in life’s activities and rewards. At Community Advantage Rehabilitation, we work together with our clients to advance their quality of life using best practices, innovation and quality standards. We are currently seeking qualified Registered Occupational Therapists  to provide services to our Adult clients within the community. This position is a flexible role, and the working hours range from part-time to full time depending on your desired client caseload size.
Provide ministry funded, community based Occupational Therapy services within the client’s place of residence
Provide occupational therapy assessment (through interview, standardized and informal assessment techniques) in the areas of home safety, home modifications, equipment prescription, fall prevention, mobility, cognition, stroke, etc
Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients/caregivers
Provide education to client/caregivers and link to community agencies/resources when appropriate
Complete and maintain documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and COTO standards
Experience in working with in a community setting is preferred but will consider new graduates
Excellent oral, written skills and report writing
Proficiency with computers
