Occupational Therapist - Satellite Health Facility – REGULAR FULL TIME

Our multi-site hospital has over 1,100 beds and we offer world-leading expertise in many areas, including cardiac and stroke care, cancer care, palliative care and pediatrics. Located in Hamilton’s downtown core, the Satellite Health Facility (SHF) team provides care for low acuity and Alternate Level of Care (ALC) patients. Generally, patients transferred from other HHS sites to the SHF are waiting for: Home with Home Care supports, Transitional Care, Long-Term Care or require time to convalesce after an acute care admission. The Satellite Health Facility operates within the Community and Population Health Services Program, under the Director of Community and Population Health Services. The SHF is comprised of full-time and part-time Nursing staff, Allied Health Professionals and Healthcare Aides supporting Inpatient services.
Position Summary As a member of the Satellite Health Facility, the Occupational Therapist will, in collaboration with the client, family and multidisciplinary team, assess individual needs, plan, and implement programs for individuals with a major focus on discharge planning and community liaison.
