implementation of Case Plan support steps with Case Management Team members and
monitoring, evaluating and mentoring interventions as
needed to actively help support youths’ progress.?
Ensuring case management support
and external service providers to meet both case management and clinical needs
of youth (eg. directly reviewing with youth, consulting with designated Consistent Workers
and others in circle of care; chairing Case Management Team review meetings.?
