TheClinical Pharmacist provides direct patient care through medicationmanagement and collaborates with healthcare teams to optimizetheuticoutcomes.

KeyResponsibilities:
  • Conductmedication therapy management (MTM) sessions with patients essingmedication effectiveness andadherence.
  • Collaboratewith physicians and other healthcare professionals to develop andadjust patient medicationplans.
  • Provideeducation to patients and healthcare providers on medicationmanagement and diseasestate.
  • Monitorpatient outcomes and make recommendations for medicationadjustments asnecessary.
  • Participatein clinical research and contribute to pharmacy practiceimprovementinitiatives.

Requirements
  • Doctorof Pharmacy (Pharm.D.) degree with residency training or equivalentexperience.
  • Boardcertification in a relevant specialty (BCPS or equivalent)preferred.
  • Strongclinical knowledge and problemsolvingss.
  • Excellentcommunication and collaborationss
