The Job Summary:

The Physiotherapist will assess and administer treatment and carryout specific physiotherapy procedures upon referral by a physician. Maintainpatient assessments, progress reports and statistical records. They will beultimately responsible to the Director of Clinical Services for providingquality care and maintaining departmental standards.

Key Accountabilities & Success Criteria:

· To assess, plan,direct, review and evaluate patient treatment on the request of the referringphysician.

· To administertreatment and carry out specific physiotherapy procedures.

· To maintainpatient progress reports and statistical records.

· To advisereferring physician(s) with regards to physiotherapy assessment and progress inthe treatment of patients.

· To liaise withother healthcare disciplines and community agencies involved in the patient’scare.

· To participate inteam conferences, presentations, educational seminars and other departmental,health centre and community programs.

· Plan a client’sdischarge in conjunction with other team members or agencies.

· Assure safeworking practices and conditions through compliance with occupational healthand safety regulations.

· Assist in themaintenance of safe equipment.

· Complies withpatient safety programs, and processes including identifying and reportingadverse events/near misses

· Attends andparticipates in appropriate safety training or education updates (ie. handhygiene, emergency code reviews)

· Identifies unsafeconditions or practices and takes appropriate action to address them includingreporting or removing hazards

· Assessment,development and implementation of a physiotherapy treatment plan for adults andolder adults with orthopedic, neurological, cardiac, post surgical and complexmedical needs

· Contributes to atransparent culture of client and staff safety by adhering to and abiding bypatient and staff safety policies and procedures set by SJHC

· Promotes anenvironment that encourages and supports change using change leadership theory

· Report unusualoccurrences and incidents promptly to the manager.

· Special projectsand other related duties as assigned by the Manager of the department.

· ThePhysiotherapist will work in the Inpatient Rehab unit

Qualifications:

· B.Sc.degree in Physiotherapy or equivalent (Registered with the College ofPhysiotherapists of Ontario

· Member in the Canadian Physiotherapy Association preferred.

Skills & Abilities:

· Superior communication and clinical reasoning skills; flexible andadaptable to changing demands and schedules

· Caring and compassionate in all interactions with Patients andFamilies

· Preferred experience in delivery of Physiotherapy Services for thegeriatric population in an integrated care setting, preferably in InpatientRehabilitation or Hospital.

· Excellent time management, organizational and prioritizationskills as relates to patient care

· Ability to demonstrate a strong customer service philosophy

· Demonstrated to be self-directed, seeking direction appropriately
