Northern College is seeking a Registered Social Worker/Mental Health Worker in our East End Family Health Team department. The successful candidate will possess excellent verbal and written communication skills along with effective assessment and problem-solving, interpersonal, leadership, and organizational skills. They will have the ability to work with minimal supervision and to establish and maintain partnerships within the social service community. A master’s degree in social work, psychology, counselling, or a related field is considered an asset.

Reporting to the EEFHT Executive Director, the successful candidate will provide client support, one on one and/or group counselling, and referral services for their patients.
What you will be doing:
• Provide social work services to patients, including system navigation, case management, and help with referrals to other programs and services;
• Provide psycho-social assessments and plan effective intervention strategies for clients with a variety of care needs;
• Provide brief individual counselling services;
• Function and operate within standards of professional competence;
• Consult regularly with members of the multi-disciplinary team regarding client status and service needs; and
• Work with clients with a variety of physical and mental health conditions.
What you will need to be considered:
• Four (4) year degree in Social Work, Psychology, Counselling Psychology, or related/relevant field;
• Registration in good standing with the Ontario College of Social Workers and Social Service Workers or other related regulatory body;
• Two (2) years of relevant experience, including
o Working knowledge of mental health legislation and related policies;
o Working knowledge of signs and symptoms of mental illness, risk indicators, and the potential impact of mental illness on family members;
o Working knowledge of the relevant services within the community;
o Must possess the ability to communicate verbally with patients, physicians, program staff, and other
stakeholders to give and receive information or instructions and to write clear and accurate reports;
o Must have the ability to function effectively as a member of a multi-disciplinary healthcare team; and
• The incumbent will be required to provide a satisfactory vulnerable sector and criminal record check.
