In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working as a member of the Fraser Health Nursing Support Services (NSS) team, the Coordinator, Nursing Support Services coordinates the provision of nursing support services under the mandate of Provincial Health Services Authority (PHSA) for children and youth (birth to 19 years) with special health care needs typically in the school setting or childcare setting. Using a family-centered approach the Coordinator performs initial and ongoing assessments; develops individual health care plans and ensures the provision and delivery of care and health supports in the identified setting. Provides consultative and educational support to non-nurse caregivers (NNC). Plans, organizes, delivers and evaluates clinical and educational programs in support of program goals. Responsibilities Receives referrals from Nursing Support Services (PHSA) Central intake via a physician-generated referral. After Central Intake triage, conducts initial assessments to determine eligibility for Nursing Support Services as per provincial guidelines. If not eligible based on the assessment, will redirect clients/families to alternative resources and supports. Evaluates and assesses child's physical, social and environmental needs; in collaboration with the family/guardian, develops and implements a child specific health care plan for delegated nursing care in the identified setting. Conducts ongoing assessments to ensure child's needs are met. Delegates nursing functions or tasks to un-regulated caregivers in accordance with BCCNM Practice Standards. Provides training to non-nurse caregivers (NNC) in the performance of delegated care procedures. NNCs typically are those who work within the school system as an Education Assistant. Monitors and evaluates care provided by the NNC. Participates in the performance and skill assessment of the NNC to ensure delivery of care in accordance with established standards of practice. Develops skill assessment programs. Certifies/decertifies NNCs specialized skills. Collaborates with patients and their families/guardians/support person, Physicians and members of the inter-professional care team in the development, implementation and ongoing modification of an optimal client care plan. Establishes a therapeutic relationship by demonstrating empathy, trust and respect and advocating for families/individuals right to privacy, dignity and access to information; ensures patient/family's choice and autonomy in decision making and care planning. Makes referrals to appropriate health care/community agencies and services relevant to their child's health care needs. Documents assessments, nursing diagnosis, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures. Participates as a member of a multi-disciplinary team in a variety of settings; acts as a clinical resource and provides nursing consultation to other team members including internal staff and partner agencies. Collaborates with internal and external stakeholders; participates on a variety of designated professional or operational committees at the program, community, regional, and/or provincial level. Collects, monitors and evaluates statistical data regarding their caseload. Provides detail for reporting purposes. Participates in the review, development, implementation and evaluation of program planning. Provides feedback to the Manager/delegate on the effectiveness of nursing support services and policy/clinical decision support tools (e.g. standards of care). Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required. Performs other related duties as assigned. Qualifications Education and Experience Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three years recent, related clinical experience in nursing, including two years experience working with pediatrics, special needs children and their families in a community setting or an equivalent combination of education, training or experience. Valid Class V BC Driver's Licence and access to a personal vehicle for business-related purposes. Skills and Abilities Thorough knowledge of the process of delegation. Demonstrated leadership and teaching skills. Ability to complete initial and ongoing client and family assessment (clinical and diagnostic reasoning) and apply nursing theory and expertise to provide nursing care pediatric clients through appropriate/prescribed technical, therapeutic, and safety interventions. Ability to use factual information, prior learning and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence informed practice. Ability to integrate and evaluate data from multiple sources to problem-solve effectively. Demonstrated ability to plan, implement and evaluate services. Ability to communicate effectively with clients, families/care givers/guardians, the public, medical staff and members of the inter-professional care team using verbal, written and electronic communication means. Ability to promote family integrated care that demonstrates empathy for client and families/care giver/guardian, sensitivity to diverse cultures and preferences, and client advocacy. Ability to prepare and present educational material. Ability to work independently and to collaborate with and promote cooperation among members of the inter-professional care team and community partners. Demonstrated ability to supervise. Ability to deal effectively with others, within a multi-disciplinary environment. Ability to establish and maintain effective community partnerships. Ability to operate related equipment including relevant computer applications. Physical ability to carry out the duties of the position.-
