Position Summary:

The Care Connector helps patients navigate through a complex health care system by connecting people to an appropriate primary care practitioner (Unattached Patient Program – UPP).

Qualifications:

Registered Practical Nurse in good standing with the College of Nurses of Ontario; Good knowledge of primary care services and other community resources in Ontario Health atHome for the area of North East, as well as the roles of health care professionals; Relevant experience in a community health care environment; Practical knowledge and understanding of relevant legislation (. regarding the provision of health care services, privacy, ; Effective planning, organizing, and evaluation skills to manage multiple clients, provide information reports, and take corrective action as required; Ability to accurately complete required documentation, reports, forms, and recommendations; Strong listening, interpersonal, facilitation and communication skills to work with diverse client groups with varying levels of comprehension and language capability; Ability to establish and maintain a wide range of contacts with health care professionals and community organizations; Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, Power Point, ; Advanced oral and written proficiency in English is essential.
