Most Canadians want to die at home, but where do you go to die when you don’t have a home? This is a question that Journey Home Hospice – operated by Saint Elizabeth Foundation – is answering for patients experiencing homelessness and structural vulnerability. With 13-beds in the heart of downtown Toronto, four beds in Windsor, and an international, teaching and learning program, we are creating specialized trauma-informed, culturally safe, and harm-reducing approaches to care that are innovating care options for people in need in Canada and beyond. Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse (RN) LEAP Certificate Required / Willing to complete LEAP Certificate within 6 months of hire Minimum 1 year Hospice Palliative care experience and ongoing continuing education is preferred Experience working with vulnerable populations Mental Health Nursing Certification or equivalent training preferred 1 year Mental Health experience and ongoing continuing education is an asset Well-developed interpersonal skills and excellent organizational skills Able to work independently as well as part of an integrated health care team (PSWs, RPNs, Physician, Nurses, Hospices Volunteers) Effective problem-solving and conflict management skills Works well in a changing environment with an appreciation for continuous quality improvement processes A current First Aid and CPR certificate is required Availability to work 12 hour shiftsAt SE, we love what we do.
