The Job Summary:

The OutpatientOccupational Therapist will provide patient care through assessment,development of individual treatment programs/consultation, direct or indirecttherapeutic interventions, evaluation of treatment and discontinuation oftreatment as appropriate and referral to other resources/services. She/he willbe ultimately responsible to the Manager of Ambulatory Care and OutpatientTherapy Services for providing quality care and maintaining departmentalstandards.

Key Accountabilities &Success Criteria:

· Be responsible for the assessment and treatment of assignedOutpatients as appropriate and to maximize their safe and independent function.

· Provide education, direction and supervision to rehab assistants,clinical OT student placements and volunteers assigned to the program, as wellas to the clients and their families and the members in the community asappropriate.

· Maintain adequate records of assessments, treatment plans,progress notes, patient attendance and other statistics, as necessary, inaccordance with departmental guidelines and the OT college of Ontario.

· Maintain liaison with medical staff, other healthcare disciplinesand community agencies involved in the patient’s ongoing care.

· Assure safe working practices and conditions through compliancewith occupational health and safety regulations.

· To assist in the maintenance of safe equipment.

· Report unusual occurrences and incidents promptly to the manager.

· The Occupational Therapist will work in the Outpatient program(Neuro and Upper Extremity)

· Adhere to provincial professional college guidelines of practice.

· Complies with patient safety programs, and processes includingidentifying and reporting adverse events/near misses

· Attends and participates in appropriate safety training oreducation updates (ie. hand hygiene, emergency code reviews)

· Identifies unsafe conditions or practices and takes appropriateaction to address them including reporting or removing hazards

· To participate in team conferences, presentations, educationalseminars and other departmental, health centre and community committees.

· Contributes to a transparent culture of client and staff safety byadhering to and abiding by patient and staff safety policies and procedures setby SJHC

· Promotes an environment that encourages and supports change usingchange leadership theory

· Improve impaired sensory-perceptual motor function andpsychosocial skills.

· Attain an optimum level of independence through treatment and useof adaptive devise.

· Attain an optimum level of physical and mental function inself-care and life skills.

· Assess the patient’s needs for an appropriate wheelchair andadaptations, provide an appropriate wheelchair and train the patient in the useof the chair (Inpatient & LTC program)

· Select fabricate and fit the patient with the appropriate adaptivedevice to meet the patient’s needs in ADL performance.

· Identify the patient’s limitations in homemaking skills and trainthe patient in such skills.

· Evaluate the patient for appropriate group activity programs.

· Plan the patient’s discharge in conjunction with other team members

· Assessment of life skills performance and architectural barriersin the home setting.

· Recommendations of necessary changes in the home environment andfacilitate safe independent functioning.

· Recommendations for following-up programs.

· Contributes to a transparent culture of client and staff safety byadhering to and abiding by patient and staff safety policies and procedures setby SJHC

· Promotes an environment that encourages and supports change usingchange leadership theory

Qualifications:

· Master’s in Occupational Therapy or B.Sc. degree in OccupationalTherapy or equivalent. Must be registered with the College of OccupationalTherapists of Ontario

· Member in the Canadian Occupational Therapy Association preferred .

Skills & Abilities:

Excellent time management, organizational and prioritization skills as relates to patient care Superior communication and clinical reasoning skills Ability to demonstrate a strong customer service philosophy Demonstrated to be self-directed, seeking direction appropriately
