Job Summary

Under the direction of the Director of Primary Care the Primary Care Nurse (PCN) respectfully works with diversity including sexual and gender minorities, and across all ethnicities and cultural, spiritual, political, age, ability, family and economic circumstances. Using a client centered, 

strength based, and harm reduction approach and with an emphasis on providing 

HIV/sexually transmitted and blood borne infection (STBBI) care, the PCN supports 

access to primary care and provides ongoing case management and comprehensive 

health care in clinic and outreach settings including community settings, shelters, 

encampments and client’s homes. 

The Pit Stop at Nine Circles is a low barrier, non-judgmental environment focused on harm reduction supply distribution and HIV prevention. It is for people who use substances, are unsheltered and who otherwise experience inequitable healthcare access.

The Pit Stop Program PCN will provide clinical leadership in the Pit Stop. They will be self-motivated, work independently and in collaboration with an interdisciplinary team. They will be responsible to develop, implement, and evaluate a proactive program for and with people who use substances to facilitate access into HIV and primary care services.

The PCN will use an outreach approach and build relationships with Pit Stop program participants, to provide in the moment, walk in HIV care and prevention, primary care, STBBI testing and wound care. The PCN will link clients to services and resources both within and outside of Nine Circles.

The PCN will be responsible for building and maintaining relationships with outside agencies, programs, and partner organizations to enhance client care and facilitate service navigation. The PCN will be a welcoming and compassionate person and will possess knowledge of social issues such poverty, food insecurity, substance use, mental health/illness, and racism.

As a Registered Nurse and Authorized Prescriber, they work to their full scope as defined by the reserved acts through organizational decision-making tools. Responsibilities also include capacity 

building of other health care providers, participation in relevant research and other 

program initiatives. 

The job is full-time, 77.50 hours per two-week period (including holiday pay hours). The regular workdays for this position will fall between 8:00 a.m. and 8:00 p.m. Mondays through Fridays. Occasional weekends may be required.

The position is classified as Group Nurse IV, on the MNU salary scale (WRHA Regional ). A comprehensive benefit package is provided as per employee eligibility via HEB Manitoba.

Qualifications

Required Education, Certifications and Experience 

Baccalaureate in Nursing from a recognized university or a combination of nursing diploma from a recognized college and clinical experience 

Registered with the College of Registered Nurses of Manitoba 

Basic Cardiac Life Support Certificate 

Certification in HIV/AIDS nursing (ACRN) (or attained within 5 years of employment) 

Registered Nurse Authorized Prescriber (RNAP) designation preferred in reproductive health, STBBI pathogens or attained within 2 years of employment. 

Knowledge, Skills, and Experience 

Minimum 3 years of nursing experience. Preference will be given to those with clinical experience in fast paced and independent practice settings such as emergency departments, intensive care units and northern nursing stations.

Demonstrated clinical skills in health assessments, physical examination, and nursing interventions

Experience in prevention, STBBI and chronic disease management 

Ability to work independently

Excellent interpersonal and communication skills 

Experience in electronic medical records and working knowledge of medical charting systems and principles 

Demonstrated computer competency and experience working with Microsoft applications including Word, Excel, Power Point, and Outlook 

Ability to work flexible hours 

Able to work effectively in a team environment 

Demonstrated ability to maintain confidential and sensitive information 

Ability to manage inter-personal conflict in a respectful manner 

Requires the ability to prioritize demands in an ever-changing, dynamic environment with attention to detail and effective organizational skills

Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families.
