Relevant du directeur, Planification et mise en œuvre - Programmes cliniques (soins primaires), le responsable est chargé de soutenir la planification, l'élaboration et la mise en œuvre d'initiatives et d'efforts au sein du portefeuille de programmes cliniques qui soutiennent l'amélioration de la qualité clinique et la transformation clinique, y compris les soins de santé primaires.

Le titulaire de ce poste travaillera en étroite collaboration avec le vice-président des programmes cliniques, le directeur et avec les équipes interdépartementales, ainsi que les responsables cliniques de la région afin de stimuler les changements de services cliniques et les améliorations plus larges du système de santé.

Planification du système de santé et amélioration de la qualité

  • Travaille en étroite collaboration avec les cliniciens et d'autres partenaires pour identifier les possibilités de rapprocher les priorités cliniques, opérationnelles et d'amélioration de la qualité dans les différents secteurs de soins de santé et domaines de programmes cliniques

  • Utilise des données et des preuves pour faciliter l'identification des domaines d'amélioration de la qualité clinique

Développement et gestion des relations avec les intervenants

  • Développer et entretenir des relations internes au sein de SO et des équipes régionales pour faire progresser les initiatives des programmes cliniques régionaux

  • Fournit des conseils et un soutien, selon les besoins, au personnel de planification de la région, aux fournisseurs de services de santé et à d'autres partenaires sur les initiatives et les opportunités qui feront progresser les objectifs stratégiques du système de santé

  • Dirige les discussions lors des réunions de service et des réunions avec les intervenants, les groupes de travail, les comités, les groupes consultatifs, le conseil et/ou d'autres réunions pertinentes qui peuvent avoir un impact sur le projet/les initiatives de l'entreprise

  • Agit en tant que représentant clé auprès de divers comités de programmes cliniques et d'autres initiatives liées au projet, selon les besoins, pour faire progresser les objectifs de l'équipe

  • Initie et consolide des relations de travail efficaces avec des partenaires externes pour se tenir au courant des avancées, des tendances et des initiatives d'amélioration de la qualité liées aux programmes cliniques

  • Travaille en étroite collaboration avec les équipes de stratégie interne, de planification ESO et de gestion des performances pour assurer la conformité au sein de l'organisation et pour intégrer et aligner les efforts à l'appui des objectifs communs

Gestion de projet et atteinte des objectifs

  • S'assure que des mécanismes appropriés sont en place pour la mise en œuvre optimale des initiatives, y compris des lignes directrices sur les meilleures pratiques et des outils pour la gestion et l'exécution efficaces des projets

  • Élabore, gère et exécute les plans de travail, y compris le suivi efficace des jalons, des risques du projet et de l'état d'avancement du projet

  • Met en œuvre une approche axée sur les données pour stimuler les opportunités d'amélioration clinique

  • Travaille en collaboration avec les ressources de soutien du projet, tant à l'intérieur qu'à l'extérieur de l'organisation dans le but d'harmoniser les processus de planification

Éducation et expérience

  • Diplôme universitaire en sciences sociales, en soins de santé, en administration des affaires ou dans un domaine connexe; une maîtrise est un atout (ou une combinaison équivalente d'études et d'expérience).

  • Au moins 5 ans d'expérience dans un environnement de soins de santé sont requis : direction de projets, développement commercial, planification du système de santé, engagement/facilitation des intervenants, services à la clientèle ou rôle similaire

  • Excellentes compétences informatiques requises (par exemple, Microsoft Word, Excel et PowerPoint, SharePoint)

  • Capacité avérée à établir, améliorer et entretenir des relations stratégiques avec les cliniciens, les gouvernements locaux et provinciaux, les fournisseurs de services de santé et d'autres partenaires clés pour travailler dans diverses équipes

Connaissances et compétences

  • Solide compréhension du système de soins de santé et du continuum de soins, y compris les processus cliniques et capacité à utiliser les données cliniques à des fins d'amélioration;

  • Capacité à penser de manière stratégique et à maintenir une vision systémique globale, et capacité à traduire des objectifs généraux en plans stratégiques et opérationnels détaillés;

Reporting to the Director, Planning and Implementation - Clinical Programs (Primary Care), the Lead is responsible for supporting the planning, development and implementation of initiatives and efforts within the clinical programs portfolio that support clinical quality improvement and clinical transformation, including primary health care.

This position will work closely with the Vice President Clinical Programs, the Director and with inter-departmental teams and clinical leaders within the Region to drive clinical service changes and broader health system improvements.

Here is what you will be doing:

Health system planning and quality improvement

  • Works closely with clinicians and other stakeholders to identify opportunities to align clinical, quality improvement and operational priorities across different health care sectors and clinical program areas

  • Utilizes data and evidence to help identify areas for clinical quality improvement

  • Supports the identification and implementation of clinical standards of care and best practices

  • Facilitates engagement with Clinical Leads to advance work plans that will achieve measurable progress in priority indicators and align with regional and provincial priorities

Develop and lead stakeholder relationships

  • Develop and maintain internal OH relationships and Region team relationships to advance regional clinical programs initiatives

  • Provides advice and support, as required, to Region planning staff, health service providers, and other partners on initiatives and opportunities that will advance strategic health system goals

  • Leads discussions in department and stakeholder meetings, working groups, committees, advisory groups, council and/or other relevant meetings which may have an impact on the project /corporate initiatives

  • Acts as the key representative at various clinical program committees, other project related initiatives, as required to advance team goals

  • Initiates and develops effective working relationships with external stakeholders to keep up to date on current/emerging developments, trends, quality improvement initiatives related to clinical programs

  • Works closely and collaborates with internal strategy, OHT planning and performance management teams to ensure alignment across the organization, and to integrate and align efforts in support of shared goals

Project management and objectives delivery

  • Ensures appropriate mechanisms are in place for the optimal delivery of initiatives, including best practices guidelines and tools for the effective management and execution of projects

  • Develops, manages and executes on work plans, including effectively tracking milestones, project risks, and project status

  • Implements a data driven approach to drive clinical improvement opportunities

  • Works collaboratively with project support resources from both within and outside of the organization to align planning processes

  • Utilizes strong knowledge of project implementation characteristics to lead large scale projects and manage communications with relevant stakeholders

  • Manage all aspects of the project life-cycle ensuring delivery on-time

Issues and stakeholder management

  • Conduct proactive issues management by anticipating and mitigating issues, facilitating discussions among partners/stakeholders

Here is what you will need to be successful:

Education & Experience

  • University degree in Social Sciences, Health Care, Business Administration or related field; a Master's degree is an asset (or equivalent combination of education and experience).

  • 5+ years experience in a health care environment required - leading projects, business development, health system planning, stakeholder engagement/facilitation, client services or similar role

  • Excellent computer skills required (e.g., Microsoft Word, Excel and PowerPoint, SharePoint)

  • Proven ability to build, enhance and maintain strategic relationships with clinicians, local and provincial governments, health service providers and other key stakeholders to work in a variety of team situations

Knowledge and skills

  • Strong understanding of health care system and continuum of care including clinical processes with ability to use clinical data for improvement required;

  • Capacity to think strategically and maintain a broad systems view, with the ability to translate broad goals into detailed strategic and operational plans required;

  • Strong ability to gather, critically analyze and synthesize data and information from multiple sources to inform health planning and to guide evidence based decision making required;

  • Ability to effectively analyze complex problems, interpret operational/clinical needs and develop integrated and creative solutions required;

  • Ability to utilize a high level of initiative and self-direction, required;

  • Strong consulting, negotiating, influencing and facilitation skills required;
