Overview The Registered Nurse (RN) role models clinical excellence and the core competencies of the practice standards of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RN works collaboratively as a member of the health care team. The RN provides direct patient care to patients based on the three-factor framework as outlined in the College of Nurses of Ontario standard, RN and RPN Practice: The Client, Nurse and the Environment. All nurses make a commitment to lifelong learning. The successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow. The successful candidate will be responsible to provide excellent care to a diverse acute medical patient population in a fast-paced setting. The Clinical Nurse Consultant (CNC) is a unique and advanced position that involves working within community-based inter-professional Palliative Care teams to ensure the delivery of effective, high quality care to patients living with a life-threatening illness and their families in their home setting. The CNC will provide expert mentorship and support to primary clinicians, nurses, and other members of the health care team to enhance palliative care knowledge and capacity within the community. As Lead Educators for Hospice Palliative Care in the community, the CNC will provide accredited education courses to health care professionals across the Ontario Health Central Region. As a member of HPC Teams, the CNC will participate in the provision of a 24/7 on call consultation service with an appropriate level of physician back up support.
