SALARY RANGE: MHEART Grid
HOURS OF WORK: 70 hours bi-weekly
This role will work Sunday to Thursday. Hours will total to 70 hours bi-weekly. You will be required to work flexible hours
STARTING DATE: ASAP
The MHEART/MHFR Float Worker will provide backup coverage for MHEART worker and will work using a collaborative crisis response model for both Sarnia Police and OPP Departments. This position will cover MHFR on days where coverage is not needed in MHEART.
MHEART
The mobile team may be dispatched by the Police Department, CMHA or BWH to complete a detailed health and mental health assessment to determine the individual’s level of risk and/or biopsychosocial needs. Based on the outcome of the assessment individuals will be provided with the most appropriate service and resources. When necessary, the mobile team will transport at risk individuals to BWH to support the need for admission and support follow up again at discharge where required.
Mental Health First Response
This position is responsible for providing responsive and comprehensive bio psycho-social assessments for individuals referred to the RAIT/MHFR team. The incumbent will gather collateral information from primary care providers, psychiatrist, family or care givers, and in coordination with the multidisciplinary team, assist in the development of recommendations and treatment planning. Brief services will be offered to individuals who require supports, within areas that have been identified through assessment.
Reporting to the Manager of Specialized Services, the incumbent will be responsible for the following functions:
Knowledge and understanding of client centred mental health and addictions interventions and practice methods
Conducting and completing detailed interRAI Assessment and synoptic report for individuals referred;
Providing crisis prevention and intervention planning with the intention of preventing a relapse and/or hospitalization
Compiling relevant case histories and gathering collateral information;
Participating in Case Disposition meetings as needed to determine outcomes for individuals referred to the Agency;
Exploring and linking individuals to external community resources to meet mental or physical health needs;
Communicating and coordinating relevant information with agency staff and police;
Responding and coordinating care with Bluewater Health Psychiatry, Emergency Department Physicians and Psychiatric Assessment Nurses.
The candidate will work with other professionals using strong problem solving and excellent communication skills. Demonstrated ability to build partnerships with community partners is required.
This position is required to obtain a Level 4 security clearance required by OPP prior to commencement of work.
