Compensation Type: Alternative Payment Arrangement. About The Role Working in a hybrid capacity, you will provide robust, comprehensive primary care to patients across a continuum of settings—street health, shelters, SROs, supported housing, and complex care housing. Care will range from episodic and opportunistic encounters to longitudinal continuity, delivered with a focus on equity and meeting patients where they are. This role involves close collaboration with embedded nursing and social work teams, often through case conferences and shared care planning. The position suits clinicians with interest in complex care, mental health, addictions, and the social determinants of health. Responsibilities Serve as Most Responsible Provider (MRP) for clients in complex care housing models. Deliver comprehensive primary care, including preventive, chronic disease, and acute care management for highly complex populations. Provide episodic and bridging care following initial nurse assessments—virtually or in-person. Develop episodic care plans with nursing staff and determine bridging to appropriate services. Collaborate with other physicians, NPs, outreach teams, and acute care colleagues to ensure continuity. Clinical activities may include:Preventive and chronic disease management (e.g., diabetes, COPD, hypertension) Management of skin and soft tissue infections (SSTIs) Bridging prescriptions for opioid agonist therapy Antipsychotic titration in cases of psychiatric decompensation Rare virtual certifications (Form 4 under the Mental Health Act) Care coordination and discharge planning, liaising with specialists as needed Qualifications & Requirements Current College of Physicians of BC (CPSBC) – Full license Canadian College of Family Practitioners (CCFP) – Mandatory Canadian Medical Protective Association (CMPA) – Mandatory MD with CCFP (or equivalent) and active license to practice in BC (CPSBC) Experience in primary care, emergency medicine, psychiatry, addictions medicine, or related fields Strong proficiency in team-based care and comfort working with nurses and social workers Familiarity or willingness to learn relevant legislative frameworks (HCCFAA, MHA, AGA) Commitment to patient-centered, harm reduction-oriented care Proficiency in EMRs (Profile, PARIS, Cerner); training and compensation provided Why Join Us Be part of an innovative care model at the intersection of housing and health Deliver equity-oriented primary care to medically and socially complex patients Work with a team committed to harm reduction, collaboration, and social medicine Contribute to a learning health system that integrates data, practice, and research Hybrid role with flexibility for virtual and outreach-based care Education, Licensing, & Experience Current CPSBC license (Full) CCFP – Mandatory CMPA – Mandatory Emergency Experience – Desired Active BC license and MD with CCFP or equivalent Experience in primary care, emergency medicine, psychiatry, addictions, or related fields Strong team-based care skills with nurses and social workers Familiarity with healthcare regulatory frameworks and patient-centered care Proficiency with EMRs (Profile, PARIS, Cerner) How to Apply Interested in applying?
