Overview We are looking for a Family physician with advanced training in Care of the Elderly to join our team in Fraser East, affiliated with Fraser Health Authority. Responsibilities Provide care to home health and homebound seniors in the Chilliwack community, including home visits and liaison with the home and community care team and the client’s family physician. Collaborate in an interdisciplinary setting; may contribute to transitional care (hospital to home) to improve outcomes and reduce readmissions. Participate in academic and teaching components of the Service and pursue a clinical faculty appointment with the University of British Columbia. Qualifications Advanced training in Care of the Elderly (certification preferred) Strong collaborative interpersonal and inter-disciplinary skills Experience in Community Geriatric consultations Eligibility to apply for a UBC Clinical Faculty Appointment Employment details Part-time position (0.2 - 0.3 FTE) Locations: Chilliwack General Hospital, Community - Chilliwack About Fraser Health Fraser Health is a health care provider serving the Metro Vancouver and Fraser Valley region.
