Key Responsibilities:
• Establish and maintain therapeutic client relationships
• Conduct thorough client assessments and screenings

• Deliver direct care and health education to clients
• Engage in collaborative care conferencing
Requirements:
• Registration as a Licensed Practical Nurse with BCCNM
• One year clinical experience with complex patients
• Valid BC Driver's license for work-related travel
• Knowledge in gerontology and health promotion
• Strong verbal and written communication abilities
