Brief Case Management / Senior Outreach and Recovery (SOAR) Clinician Direct Service Regulated - (OPSEU) Full-Time, Contract (Approximately 12 months) Woodstock, Ontario (Oxford Zone) Application Deadline: December 14, 2025 The Case Management Clinician provides comprehensive mental health and addictions support to adults and older adults (65+) within a multi-disciplinary team. This split role combines general case management for individuals with severe and persistent mental illness and co‑occurring disorders, with specialized psychogeriatric care for seniors experiencing mental health challenges, dementia, and related conditions. Working from a trauma‑informed, recovery‑oriented perspective, the clinician co‑develops individualized recovery plans, delivers therapeutic interventions, and advocates for client rights while fostering community integration and harm reduction. Apply recovery principles and the bio‑psychosocial model of care. Promote client dignity, welfare, and rights at all times. Maintain a professional, client‑centered, trauma‑informed approach. Establish therapeutic relationships and collaborate with clients, families, and care providers. Utilize Motivational Interviewing and evidence‑based practices for mental health and addictions. Maintain knowledge of community resources and facilitate linkages. Direct Service (Assessment, Crisis Support, and Follow‑up) Manage a caseload of adults and seniors with complex mental health needs. Conduct standardized screenings, assessments, and care planning. Engage in collaborative care models with community partners (attend hospital rounds, psychiatry, consultations, etc.) Provide individual and group interventions, including facilitation and psychosocial programming. Deliver psychoeducation on medications, symptoms, and coping strategies. Support skill development, resource navigation, counseling, and advocacy. Implement crisis prevention, intervention, and safety planning. Monitor symptomology, medication adherence, and daily functioning. For seniors: provide psychogeriatric consultation, dementia care interventions, and support for care partners. Utilize frameworks such as P.I.E.C.E.S and conduct MoCA assessments. Employ hybrid service delivery (in‑person, phone, video conferencing). Act as liaison among clients, families, and multidisciplinary providers. What you’ll need to apply Police Vulnerable Sector Check Requirements – Successful candidates must have a Police Vulnerable Sector Check completed within the last six months. Applicants residing in the City of London must obtain a PVSC promptly as processing can take approximately two weeks; offers cannot be made without a PVSC. Qualifications: RPN, RN, or Bachelors/Master’s in Occupational Therapy, Social Work, Psychology, Nursing, or a related field (primary proof of credentials required during the hiring process). Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers, the College of Nurses of Ontario, the College of Registered Psychotherapists of Ontario, or the College of Occupational Therapists of Ontario. Valid Driver’s Licence and access to a vehicle with at least $2 million in liability insurance to transport clients. Preferred qualifications Several years working with individuals with serious mental illness and addiction. Community‑based service experience and knowledge of local support networks. Crisis intervention and resolution skills. Experience with ABI, developmental disabilities, and seniors’ mental health. Familiarity with P.I.E.C.E.S framework and MoCA assessments. Experience facilitating therapeutic groups. Psycho‑social rehabilitation principles. Pharmaceuticals used in mental health treatment. Regional mental health and social service networks. Legal system and seniors‑specific care considerations. Strong assessment, planning, and organizational abilities. Crisis management and intervention skills. Excellent communication and relationship‑building skills. Computer proficiency and documentation accuracy.
