Key Responsibilities: • Develop and document person-centered support plans • Provide direct care to medically fragile individuals • Encourage individual rights and community engagement • Monitor individuals’ health and behavior changes • Maintain clear documentation and communication
Requirements: • Minimum 2 years’ experience with disabilities and complex needs • Diploma in a relevant discipline preferred • Strong written and verbal communication skills • Effective coaching and mentoring abilities • Teamwork and adaptability skills
