Wesley seeks an Intensive Case Manager for Housing Services to empower vulnerable families in Halton. This hybrid position combines direct support with administrative responsibilities. As a part of a respected non-profit, you will deliver vital services to homeless individuals, fostering their transition to secure housing. This role demands strong communication skills and a deep understanding of diverse client needs, working collaboratively to create effective housing plans. - Facilitate intake and develop housing plans for clients
- Address referrals and inquiries from community agencies
- Assist clients in securing appropriate housing
- Screen clients for domestic violence history
- Conduct regular home visits to support life skills
- Degree/diploma in Human Services or related field
- Good standing with Ontario College of Social Workers
- Proven experience with mental health and homelessness
- Willingness to work flexible shifts
- A valid driver’s license is an asset
