Deliver clinical pharmacy care in patients' homes with VHA Home HealthCare, combining medication expertise with patient counselling and follow-up.
This Pharmacist practices across VHA's newly established pharmacy and community settings. You will review prescriptions, resolve drug-related problems, and collaborate with patients, caregivers, primary care providers, and healthcare professionals. The work supports safe medication use and client-centred care.
Key Responsibilities:
- Review and verify prescriptions before medication distribution
- Complete medication reconciliation, consultation, counselling, and follow-up
- Develop and monitor medication care plans
- Contribute to interdisciplinary care rounds
- Advise teams on drug therapy and therapeutic outcomes
Requirements:
- Pharmacy degree from an accredited post-secondary program
- Registration and good standing with Ontario College of Pharmacists
- Ability to perform technical medication dispensing duties
- Critical thinking skills for independent client care
- Commitment to client-centred care and health teaching
