What The Role Involves Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Home and Community Care Support Services (HCCSS).Working with patients (both in-person and virtually) to provide exceptional nursing servicesProviding services one-on-one with patients in a home settingCommunicating and liaising with patients, family members and others within the interdisciplinary care teamPerforming other clinical duties as required What You Bring Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma Current College of Nurses of Ontario (CNO) registration in good standingA strong desire and commitment to making a difference in the lives of our patientsExceptional interpersonal skillsA valid driver's license and vehicle as regular travel is requiredExcellent English verbal and written communication skillsCPR CertificationCurrent immunization record including 2 step TB test To protect our patients, we require a current vulnerable sector check and a clear background check CarePartners In Your Community In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
