Position Summary

The Social Worker identifies the need for the delivery of Social Work services, recommends interventions and participates in the development of care plans to meet patient's needs. The social worker will help to bridge care transitions from the inpatient setting to the outpatient setting while providing patient's and their families with support throughout the continuum of care.



Required


• Current Certificate of Registration, or eligible for registration with the College of Social Workers (must register within the first 2 months if position offered)

• Master's Degree in Social Work

• At least one-year recent related experience in grief counselling or related area.

• Experience in conducting psychosocial assessment, de-escalation and brief psychosocial interventions.

• Project planning/development considered an asset.



Duties


• Crisis response, mental health assessments, interventions and consultations for adults, and families in the outpatient rehabilitation setting

• Develop appropriate short-term and long-term treatment plans to meet client(s) needs.

• Provide & receive consultation to the multidisciplinary team, peer supervision meetings, client's families, physicians, agencies and police services.

• Keep current on all aspects of social service agencies & community mental health programs.

• Participate in Departmental CI on a continuous basis.

• Help develop foundation for outpatient services through outpatient rehabilitation department

• Work collaboratively in outpatient setting while also creating rapport with inpatients who will be referred to the outpatient program

• Attend inpatient rounds on Sills 3 once per week

• Must be available to work shifts, weekends and holidays.

• Must be available for call-in on short notice.
