Position Summary

The Float Field Case Manager (FCM) provides flexible coverage across multiple territories and physician offices, stepping in wherever support is required. This role is not assigned to a fixed caseload; instead, the Float FCM manages caseloads across the broader area to maintain consistent service delivery during periods of vacancy, high volume, or staff leave.

Working closely with prescribing physicians, patients, and the PSP team, the Float FCM enrolls patients into the PSP, coordinates reimbursement needs, provides clinical and educational support, and ensures timely access to therapy and end-to-end support as needed. The role requires strong clinical judgment, exceptional adaptability, and the ability to respond quickly to shifting operational needs.

Primary Duties and Responsibilities

Practice Management & Clinical Skills

  • Adheres to regulatory college standards and applicable clinical practice guidelines.
  • Guides physicians and patients through PSP enrollment and program expectations.
  • Conducts comprehensive patient assessments, reviews medical histories, and evaluates disease activity.
  • Collaborates with prescribing physicians to complete enrollment and define next steps in the patient journey.
  • Partners with non-clinical case managers to complete special authorization and public payer forms or completes end-to-end processes as needed.
  • Anticipates barriers to coverage and offers recommendations to optimize outcomes.
  • Maintains accurate and thorough documentation in the client management system.

Patient Support

  • Initiates enrollment calls, gathers patient information, and provides education on therapy and next steps.
  • Keeps physicians informed of patient file progress and anticipated timelines.
  • Coordinates services in alignment with program KPIs to ensure timely therapy access.
  • Works with reimbursement teams to manage public and private coverage requirements.
  • Supports patients with pharmacy selection, delivery coordination, and ongoing therapy monitoring.
  • Reports Adverse Events in accordance with program procedures.
  • Identifies systemic coverage barriers and escalates trends to leadership.

Flexible Deployment & Coverage

  • Provides coverage across multiple regions as assigned based on operational needs.
  • Rapidly integrates into varied physician offices, adapting to different workflows and expectations.
  • Supports caseloads during vacations, vacancies, leaves, or surges in patient volume.
  • Maintains readiness for frequent travel and changing schedules.

Mentorship & Peer Support

  • Assists in onboarding and training new Field Case Managers.
  • Shares best practices and acts as a resource for peers across regions.
  • Contributes to a collaborative and supportive PSP team dynamic.

Client Management & Physician Interaction

  • Conducts in-person visits with physicians and collaborates with Sales Representatives when required.
  • Establishes strong working relationships with physicians and office staff across multiple territories.
  • Provides regular updates on patient status and program processes.
  • Helps resolve client–physician concerns through proactive communication.
  • Acts as a conduit for field insights, informing clients of emerging trends or challenges.
  • Responds to physician and patient requests in a timely manner as outlined by program expectations.

Experience and Educational Requirements

  • Licensed healthcare professional (RN, RPN, LPN, NP, Pharmacist, or Respiratory Therapist) in good standing.
  • Valid professional liability insurance, renewed annually.
  • Minimum three (3) years of clinical experience an asset.
  • Ability and willingness to travel extensively across the region.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Experience in Gastroenterology, Dermatology, Rheumatology, Respirology, Immunology, or Allergy an asset.
  • Knowledge of public and private reimbursement frameworks an asset.
  • Experience with specialty medications/biologics preferred.
  • Proficiency with computer systems and digital tools.
  • Bilingualism (French/English) an asset.

  • Strong organizational skills and reliability in meeting deadlines.
