• Paid mentorship opportunities

• Participation in a self-scheduling, paid, after-hours care system (commitment of only one week of coverage per quarter; coverage is shared amongst the Vancouver LTCI practitioners)
• Opportunities for paid participation in long term care-specific events

• Opportunities to participate in LTC quality improvement projects and CME

• You can develop and grow your skills in providing care for the frail elderly and transfer these skills to your work in community and hospital settings

• Allows for flexibility in your schedule – supplement other family practice work with a LTC panel to add variety to your workday. You can work with the care home leadership to set the size of your panel and your availability for providing care
• Caring for the frail elderly is both challenging and incredibly rewarding

Job Specifics

The Vancouver Division of Family Practice LTCI enables physicians who work in long term care and commit to a formal relationship with a care home to receive additional funding. Physicians will form part of a supportive physician community that work toward maintaining the five best practice expectations including: regular proactive visits; on-site medication reviews; attendance at care conferences; involvement in 24/7 on call; and completed documentation in charts.

Remuneration: LTCI care home panels are remunerated through the LFP model or fee-for-service. Physicians who join the LTCI receive payments for:

• Participation in the after-hours care program: $1,000 for weekday shifts (5pm to 8am Monday to Thursday) and $1,400 for weekend shifts (5pm Friday to 8am Monday)
• Sessional payments for participation in mentorship activities, events, and quality improvement projects

Windermere Care Centre is a 209-bed long term care home currently being supported by Six Long Term Care Initiative (LTCI) practitioners. One practitioner is leaving and therefore an additional practitioner is required to assist with the resident caseload. Practitioners must have, or be willing to obtain, Vancouver Coastal Health (VCH) privileges and credentials to work at this care home.
