Role Overview

The Health & Wellness Coach is a licensed nurse (LPN) who blends clinical expertise with coaching to promote autonomy, dignity, and wellness. This role supports individuals in managing health needs as part of their life redesign journey, while mentoring team members and integrating health strategies into daily routines.

Key Responsibilities
  • Co-create individualized health strategies that promote independence and participation
  • Monitor health needs and adapt supports over time
  • Coach team members and families in applying wellness strategies
  • Translate clinical insight into practical, empowering practices
  • Ensure documentation aligns with evolving health goals
  • Promote safety and regulatory compliance
  • Participate in planning, reflection, and on-call support
Qualifications
  • Registered/licensed and in good standing with CNO (Ontario)
  • Experience supporting individuals with brain injury, stroke, or complex health needs
  • Strong coaching, mentoring, and relational skills
  • Commitment to person-centered, strengths-based practice
  • Comfortable working outside traditional institutional models
Requirements
  • Criminal Record Check (Vulnerable Sector)
  • First Aid with CPR
  • Food Safe
  • Current TB Status (within 30 days)
  • Immunization Declaration
  • Valid Driver's License and Abstract
Working Conditions

Flexible scheduling with on-site presence. Some remote documentation tasks (with approval). On-call support component included.
