Position Overview
Reporting to the Community Outreach Lead, the SUN Housing Support Worker implements a Housing First approach, focusing on reducing barriers to housing, preventing homelessness, and supporting sustainable tenancy. The Housing Support Worker engages with individuals who are experiencing homeless or at risk of homelessness, assisting them in identifying and meeting their housing and other needs. This role aligns with the YMCA Mission, Vision, and Core Values and incorporates Reaching Home objectives
Duties
· Engage visibly homeless people living in public places, hidden homeless and those at risk of homelessness. The Housing Support Worker is expected to meet people outside, making exposure to the weather an expectation of the position.
· Perform an intake and assessment of each client to assist in determining the "readiness" of each client
· Develop case management plans for each client
· Make necessary referrals, as required (i.e. Income assistance, NB Housing, mental health etc.)
· Develop relationships/network with a variety of people and organizations
· To provide ongoing support in obtaining and maintain tenancy
· Life skills training
· Responsible for establishing a committee made up of community partners that will work collaboratively to support clients transitioning into housing
· Responsible for maintenance of statistics and report writing
· In some cases, accompany clients to appointments or other safe places
· Advocate on behalf of homeless individuals in accessing appropriate services such as health care and financial supports
· Engage in crisis prevention/intervention with clients
· Responsible to attend all staff meetings, department meetings, and professional development opportunities as deemed necessary by management
· Build and maintain relationships with community partners.
·Other duties as assigned.
Skills & Qualifications
- A criminal record check and vulnerable sector check are required as a condition of employment
- Valid drivers license & Access to a vehicle on shift is a required condition of employment
- Able to work flexible hours
- Completion of college or university; Social Service Worker (SSW) Diploma, Community Support Worker (CSW) Diploma or Human Services Diploma, OR related work experience
Job Types: Full-time, Permanent
Pay: $40,000.00-$42,000.00 per year
