Purpose of the Position:
The Inner City Family Health Team is a multidisciplinary team which serves homeless clients with complex health needs. These include untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental health and substance use challenges. Services are provided by the team using a holistic, client-centred model of care with an emphasis on disease prevention, chronic disease management, harm reduction and health promotion.
The Social Worker collaborates with Inner City Family Physicians, Nurse Practitioners, the interdisciplinary team, and the community to provide comprehensive health services, counselling, engaging in psychosocial assessments, service navigation, harm reduction, education, advocacy, and support to clients experiencing mental health and concurrent substance use challenges.
Duties & Responsibilities
· Provides individual and group counselling for a range of mental health and/or addiction-related needs, including anxiety, mood disorders, PTSD and improving coping skills using a range of therapies such as DBT, CBT, trauma-focused, mindfulness-based and psycho-education interventions.
· Assists clients in identifying and working toward their goals using a holistic, client-centred harm reduction and recovery model. Encourages client involvement in health care decisions and in taking action for their own health.
· Participates in interdisciplinary team rounds/case conferences. Identifies cases for presentation at Team meetings for case review/case management.
· Assists with Risk Management by providing harm reduction supports, assessing and identifying signs of deterioration of mental health, mobilizing resources and intervening to prevent the onset or prolongation of crisis.
· Collaborates with clients, physicians, and other allied health members of the interdisciplinary team to consider and develop appropriate, client-centred, individualized care plans with the client and other caregivers based on best practices.
· Coordinate referrals and access to health care and community resources, advocacy and complete applications/forms in support of client goals (e.g., housing, medication coverage, CPP).
· Consults and communicates effectively with internal and external providers to ensure cohesive, integrated care.
· Contributes to the development and implements strategies that promote health, prevent disease, and foster independence and healthier lifestyles for Inner City clients.
· Undertakes home visits and outreach services as required and appropriate.
· Develops, leads, and evaluates group programs related to recreation, isolation, or therapy.
· Maintains and develops professional competence through ongoing professional development and maintains current discipline-specific standards of practice.
· Stays current and aware of opportunities to implement new evidence-based methods of client assessment, treatment and programming.
· Performs additional administrative tasks and other duties as assigned.
Qualifications, Skills and Experience:
· A Master's degree in Social Work.
· At least 3 years of experience providing direct social work support to individuals from marginalized communities including those who experience homelessness, substance use, mental health, poverty and racialized communities.
· At least 2-3 years of experience providing individual mental health counselling, including evidence-based therapeutic interventions such as DBT, CTB, CTI, mindfulness, etc.
· Experience working with homeless clients is preferred.
· Clinical experience working in or collaborating with community-based agencies.
· Extensive knowledge and experience working from a harm reduction perspective.
· Knowledge of and commitment to client–centered philosophy and principles and practice of community service delivery and chronic disease management models.
· Knowledge of community resources relevant to the provision of mental health services, harm reduction, health promotion and illness prevention.
· Working knowledge of the Ontario Mental Health Act, Healthcare Consent Act, Substitute Decisions Act and Community Mental Health resources.
· Knowledge and experience in the application of electronic medical records (EMR) preferred.
· Ability to maintain professional practice regarding commitments to colleagues and clients, pertinent legislation, evidence-based standards of care and professional standards of practice.
· Registration with the Ontario College of Social Workers and Social Service Workers.
