Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality care for residents in long-term care. This role requires a strong understanding of medical procedures, patient needs, and effective communication to ensure the well-being of our patients. The LPN will be responsible for delivering safe, efficient, and personalized nursing services in accordance with state regulations and facility policies.
Duties
- Administer medications and injections accurately following physician orders and facility protocols
- Monitor patient vital signs and assess their condition regularly
- Assist with patient hygiene, mobility, and comfort measures
- Document patient care activities using electronic health records
- Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement care plans
- Educate patients and families about health conditions, medication management, and post-discharge care instructions
- Manage cases involving dementia care/senior care
- Ensure compliance with infection control protocols and safety standards at all times
- Maintain accurate documentation of all patient interactions and interventions
Experience
- Prior experience in nursing home settings or hospital environments is highly preferred
- Familiarity with medical terminology, medication administration, and patient documentation systems such as Epic or Cerner is essential
- Background in assisted living or senior care enhances the ability to deliver specialized services for elderly populations
- Knowledge of pain management techniques and case management practices is beneficial
- Previous roles involving patient care in medical offices or nursing facilities will support success in this position
