What you'll be doing: 

  • Conducts comprehensive nutrition assessments for patients.
  • Develops and implements individualized nutrition care plans
  • Provides counselling and education and in-services to patients, using trauma-informed and patient centered approach
  • Documents as per HHC and college standards, including assessments, interventions and outcomes in patient medical record
  • Collaborates with the clinical team to provide the best nutritional care for patients
  • Advises and develops custom menus, snacks
  • Promotes healthy eating behaviors 

What we're looking for: 

  • Bachelor's degree in Human Nutrition and Dietetics from an accredited post-secondary institution
  • Valid registration with College of Dietitians of Ontario is required, member of Dietitians of Canada preferred
  • 5 years' previous experience in Older Adult, and Eating Disorders 
  • Experience with mental health and addictions populations 
  • Collaborative, service-oriented, innovative and accountable
  • Commitment to working within Homewood's Health and Safety Standards
  • Practice in accordance within the ethical guidelines of the Dietetics profession
