Job Description
The Trillium Health Partners (THP) Regional CKD Program supports one of the fastest growing regions in need of dialysis within the province. To support our community, patients and families, the program has developed a new leadership structure that ensures leadership presence and coordination of care across all sites and modalities. The Regional CKD program is responsible for pre-dialysis, chronic outpatient and home dialysis and acute dialysis care for patients within the Mississauga and South Etobicoke region.

Reporting to the Patient Care Managers (PCM) of the Regional Chronic Kidney Disease Program, The Clinical Nurse Supervisor is a fundamental component of the leadership triad working in collaboration with the PCM and Educator to ensure safe and high quality care for patients within all modalities across THP and the Regional CKD Program.

The Clinical Nurse Supervisor is responsible for the oversight of nursing, clerical staff, and renal assistants. They are instrumental in supporting the inter-professional team through daily operations and works collaboratively with the leadership team to plan, develop and implement best practices that guide patient care, workflow and efficient operations. In addition to routine daily operations, the supervisor will work with staff, physicians, patients and families to monitor patient safety, mitigate risk and implement quality improvement initiatives.

Duties And Responsibilities

  • Ensures the smooth operations of the CKD program by ensuring daily operational procedures are followed, identifying opportunities for improvement in the unit and collaborating with the Renal team to implement and sustain process changes as needed reviewing and supporting daily scheduling requirements; forecasting staffing and scheduling needs; coordination of nursing and other care team resources; ensuring equipment and supplies are adequately maintained to support efficient patient care; adherence to policies, procedures and standards of care; continuously monitoring patient flow and capacity
  • Collaborates with the Education team in the creation and implementation of policies; orientation, training and staff development and investigating and resolving patient safety events
  • Conducts investigation and identifies appropriate resolution for patient, family, or staff concerns
  • Ensures the ongoing success of the department by: anticipating and minimizing organizational and unit based barriers; setting performance goals for achieving maximum scope of practice; demonstrating effective working relationships; and partnering with the Patient Care Managers and the Education team to ensure a collaborative work setting
  • Supports and maintains the priorities of the CKD Program, as well as activities to support ORN initiatives
  • Provides coaching and mentoring to members of the Renal team
  • Conducts performance management activities in consultation with Patient Care Managers and Human Resources
  • Actively participates in recruitment and onboarding process for nursing, clerical, and renal assistant staff
  • Collaboratively plans, implements, and evaluates patient safety and quality improvement initiatives; ensures strategies to sustain initiatives
  • Monitors and analyzes fiscal resources; supports data collection requirements
  • Provide leadership coverage to all areas of the program, including Renal clinics, home dialysis, and acute dialysis modalities, across all sites of Trillium Health Partners, on an as needed basis

Key Qualifications

  • Must have a current registration with the College of Nurses of Ontario
  • Registered Nurse with a Baccalaureate degree in Nursing is required
  • Master's Degree is an asset
  • CNA certification; CNeph(c) is an asset
  • 3-5 years in nephrology nursing is an asset
  • Minimum 2 years supervisory/leadership experience in a health care environment is required
  • Demonstrated knowledge of hospital nursing functions and current trends in nursing and health care
  • Experience with Ontario Renal Network initiatives an asset
  • Knowledge of and ability to work with Collective Agreements and Labour Relations issues is an asset
  • Proven flexibility, initiative, independent judgment and ability to be self-directed within scope of responsibility
  • Experience in process improvement and change management strategies
  • Demonstrated strong organizational skills with ability to prioritize and multi-task
  • Strong interpersonal, communication, negotiation and conflict management skills
  • Access to a car and ability to travel between sites is required
  • Satisfactory performance and attendance record is required
