Position Overview
As a member of the department of pharmacy team, you are responsible for the day to day activities related to the responsible provision of medication therapy management including provision of pharmaceutical care, teaching professional students and staff,supervision of technical personnel and distribution activities.
Currently available roles are: palliative care/float, inpatient surgery/float
Primary Responsibilities
PHARMACEUTICAL CARE/CLINICAL SERVICES:
- To provide direct patient care in the management of drug therapy to achieve positive patient
health outcomes in medicine, surgery or critical care, and coverage to other patient care areas as
needed. Direct patient care focuses on electronic verification of medication orders; selection and
assessment of patients requiring pharmaceutical care; identifying, resolving and preventing drug
related problems; patient education; electronic medication reconciliation; continuity of patient care;
and the provision of drug information.
- Support daily drug distribution activities, including weekend, evening and oncall assignments
- Support and participate in educational programs and projects sponsored by the pharmacy discipline or hospital, specifically teaching pharmacy students and other professional staff; as well as continuing education and professional development.
- Participate in the development, revision, and implementation of standards, protocols, procedures, guidelines and programs pertaining to drug therapy and pharmacy practice.
EDUCATIONAL SERVICES/PROFESSIONAL DEVELOPMENT:
- Provides and/or participates in the following services to facilitate the indirect delivery of
pharmaceutical care:
- Provides inservice lectures and/or written information to assigned nursing units and medical staff
as required.
- Provides drug information support to health care professionals and hospital committees.
- Participates in the development and implementation of pharmaceutical care.
- Prepares Pharmacy Bulletin as scheduled.
- Gives clinical presentations as scheduled.
- Assumes the duties of preceptor or clinical tutor for pharmacy students and interns as needed.
- Maintains a current continuing education certificate by attending or participating in some/all of
the following activities:
- Grand Medical and specialty Teaching Rounds
Clinical sharing sessions/Journal club/Clinical presentations
External continuing education programs, including organizing or giving presentations on
request.
DRUG DISTRIBUTION SERVICES:
- Supervises pharmacy assistants in their dispensary duties.
- Responsible for investigational, narcotic and controlled drug inventory control and records.
Resolves discrepancy issues and ensures safe destruction of expired stock.
- Maintains an efficient workflow and ensures that medications are sent to the patientcare areas
on time.
- Meets with Nurse Manager of assigned units to review the drug distribution system to improve
effectiveness. This includes supervision of regular medication roomaudits.
- Reviews assigned unit's floor stock medication lists on a regular basis.
- Responsible for developing manufacturing and pre-packaging procedures, expiry dates and
records as assigned.
- Supervises Pharmacy Assistants during manufacturing and pre-packaging processes.
- Assists with dispensing when required.
INTRAVENOUS ADMIXTURE SERVICES:
- Supervises Pharmacy Assistants performing the technical functions of the IV Admixture program, including TPN, chemotherapy and sterile manufacturing, when applicable.
- Ensures safe procedures are followed.
- Monitors TPN therapy.
- Participates in the I.V. quality assurance program.
- Maintains an efficient workflow and ensures that medications aresent to the patient care areas
on time.
- Ensures stability of I.V. medications and updates I.V. compatibility and stability data as required.
- Assists with I.V. admixture preparation and dispensing as required.
ADMINISTRATIVE/OTHER:
- Reports any medication discrepancies to the Director or Manager, Pharmaceutical Services;
identifies corrective action and documents as a medication incident if needed.
- Participates in the adverse drug reaction reporting program.
- Responsible for training and orienting new pharmacists, pharmacy students and pharmacy assistants.
- Participates in supervision and control of the hospital formulary system.
- Participates on departmental, hospital and externalprofessional committees as required.
- Orientates nursing personnel to Pharmacy policies and procedures, as required.
- Participates in hospital Quality Improvement and Risk Management programs.
- Responsible for workload measurement activities associated with the assigned duties.
- Observes all departmental and hospital Occupational Health and Safety requirements and
participates in departmental inspections.
- Attends departmental conferences or reads the minutes when absent.
- Rotates as facilitator/recorder at pharmacist and staff conferences.
- Is available to work week-end and evening shifts and assists in completion of the calendar schedule.
Education
- Bachelor of Pharmacy degree or Doctor of Pharmacy.
- Registration as a Part A Pharmacist in Ontario.
- Pharmacy Examining Board of Canada certification.
- Hospital Pharmacy residency certificate or equivalent experience preferred.
Experience
- Clinical Hospital Pharmacy patient-focused experience (preferred minimum 1 year).
- Clinical oncology experience an asset
- Excellent communication skills for teaching professionals, students and patients.
- Ability to organize and supervise the work of technical personnel.
- Exceptionalprofessionalism and abilityto maintain composure under stress.
- Innovative and creative in approach to problem-solving.
- Interacts courteously and efficiently in verbal and written communication.
- Working knowledge of pharmacy and/orhospital technological solutions including CPOE, eMAR, electronic documentation, Pharmacy Information Systems (i.e. Cerner)
- Demonstrated leadership ability.
- Demonstrated ability to plan, coordinate and execute professionalresponsibilities.
- Excellent work andattendance record required.
- CSHP membership/participation an asset (or equivalent).
