Job Description: The Assessor Coordinator is one of 3 positions that will report to the Home Care Manager NW5 Wilkie, Unity and Macklin area and work in partnership with the Nurse B – Home Care Nurse Unit Coordinator. The Assessor Coordinator is accountable for the day to day assessing, planning and coordinating referrals and services for home care referrals and hospital-based clients. The Assessor Coordinator ensures the transition from hospital to discharge occurs in as timely a manner as possible.

Experience

  • Minimum of 1 year nursing experience in Acute or Home Care Setting

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Registered Psychiatric Nurse license is acceptable
  • Practical Nursing diploma with 4 years experience is acceptable
  • Bachelor of Psychiatric Nursing is acceptable
  • Psychiatric Nursing diploma is acceptable
  • Nursing diploma is acceptable
  • Current registration w/ Registered Psych. Nursing Assoc. of SK (RPNAS) required

Knowledge and Abilities

  • Ability to work independently and as a leader of a multidisciplinary team
  • Ability to assist and develop policies and procedures according to best practice
  • Makes judgements on the basis of critical thinking and problem solving criteria
  • Strong analytical and problem solving skills
  • Ability to deal tactfully, non-judgmentally and effectively with people
  • Requires sound knowledge of nursing process
  • Must be well organized and able to meet deadlines
  • Must be culturally sensitive when working w/ multicultural clients and coworkers
