The Family Birthing Centre Social Worker is a highly skilled specialist dedicated to supporting families across the perinatal continuum. This healthcare professional is a key part of the interprofessional team and provides compassionate emotional support while expertly coordinating access to essential resources for parents and newborns. With extensive experience navigating complex psychosocial circumstances, the FBC Social Worker plays a critical role in promoting optimal health and positive outcomes for families during pregnancy, birth, and the postpartum period. This social worker is deeply committed to delivering equity focused, culturally competent and family centered care. The Family Birthing Centre Social Worker possesses advanced expertise in perinatal mental health, risk assessment as well as grief and bereavement counselling and support. As a strong advocate for families, this professional demonstrates a clear commitment to continuous quality improvement through teaching, research and the development and facilitation of education initiatives focused on perinatal health. This position works in partnership with the Social Worker in the NICU to support a seamless care experience for families requiring a higher level of care after birth.

Duties & Responsibilities
Social workers in health care provide in-depth biopsychosocial assessments and implement solution focused care plans for individuals and families, solution-focused counseling, discharge planning, resource referral, consultation and collaboration, teaching, research/quality improvement, and patient and family education are required elements of the position. In addition, Social Workers at Unity Health Toronto:

  • Work effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient care
  • Contribute to the academic mandate of the organization as an educator, resource or support for staff and members of the academic and professional community
  • Identify and address educational needs for patients and their families and the interprofessional team
  • Demonstrate a commitment to ongoing professional development in order to continue to provide optimal patient care
  • Are committed to and participates in ongoing quality improvement, policy development and/or research in the organization
  • Provide patient care in accordance with all professional and organizational standards
  • Are committed to providing anti oppressive and accessible care for all patients

Qualifications

  • Must be currently licensed by and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas
  • Must hold a Master's Degree in Social Work
  • Current BLS (or willingness to complete within probationary period)
  • Experience in a children and family serving agency or hospital social work in women and children's health, preferred.
  • Experienced in completing bio psycho social assessments for patients with a diverse range of needs including those new to Canada, patients impacted by mental health and substance use, medical complexity and with historical or current child welfare involvement.
  • Able to complete assessment of risk and provide solution focused, brief interventions for patients and their families during their admission to the Family Birthing Centre
  • Excellent communication, critical thinking, and interpersonal skills
  • Proven flexibility and adaptability in a fast-paced environment
  • Excellent time management, organizational and prioritization skills as relates to patient care
  • Demonstrated advocacy, commitment to and involvement with profession preferred (e.g. involvement with professional association, representation on relevant committees)
