Principal Engineer (Path To CTO) — Blair Health

Blair Health is closing the specialist gap in women's healthcare by making expert menopause and midlife care instantly accessible using AI-driven clinical intelligence. We're hiring a Principal Engineer — a hands-on technical leader who can architect, build, and scale the platform while pushing the frontier of LLMs, ML, and clinical decision-support systems.

What You'll Do

  • Lead the technical vision and architecture across our full stack (React/TS, Tailwind, Node/Python, Supabase/Postgres)
  • Design and build scalable microservices and secure, compliant infrastructure (HIPAA/PHIPA)
  • Drive our AI/ML/LLM innovation roadmap, working with clinicians on real diagnostic and personalization workflows
  • Ship fast: build end-to-end features for patients, providers, and operations
  • Set engineering standards, DevOps practices, API strategy, and quality benchmarks
  • Optimize performance, reliability, and security as we scale across Canada & the U.S.
  • Partner directly with the CEO and clinical co-founder on product direction and technical strategy
  • Recruit, mentor, and grow the engineering team from 0 → 10
  • Operate like a founding technologist with material early-stage equity

Experience We're Looking For

  • 5–6+ years full-stack engineering experience (startup or fast-paced environment ideal)
  • React/TypeScript + or Python expertise
  • Strong background in cloud (AWS/Vercel), APIs, and microservices
  • Experience with Postgres/Supabase or similar relational systems
  • Exposure to ML/LLM systems, embeddings, or applied AI (or strong desire to learn quickly)
  • Experience building production systems end-to-end with high reliability
