THE OPPORTUNITY - Housing Services Practitioner - Complex Needs

Reporting to the Housing Services Team Leader, the Housing Services Practitioner (Complex Needs) supports persons with disabilities, who have complex needs, to live as independently as possible in the most appropriate living environment consistent with Vecova's Mission, Vision, Values and Guiding Principles. This includes ensuring consumers are safe and involved in decisions and activities so they can be successful in their home and community.

This position provides direct support to persons with a disability with a medical or mental health diagnosis. Hours of work and location are dependent on the needs of the consumers, team and Vecova's organizational needs.

WHAT YOU WILL DO

  • Housing Support Services
  • Use person-centered approaches to assist and support consumers to achieve their housing goals.
  • Provide all consumers support in accordance with Vecova training, policy and procedures
  • Consumer Support tasks, additionally knowing the consumers' Planned Approaches
  • Understand and follow all safety standards for the consumers' service and home as outlined in policy & procedures
  • Planning and Communication for consumer needs/changes
  • Work effectively within culturally diverse teams
  • Demonstrate a high standard of professionalism aligned with Vecova values
  • Maintain a positive working relationship with all Vecova team members
  • Represent Vecova in a professional manner
  • Demonstrate a high level of initiative and enthusiasm
  • Maintain confidentiality at all times
  • Demonstrate a high level of administrative and IT capacity

WHAT YOU BRING

We are looking to engage team members with diverse skills, experiences and interests. These are some of the competencies and credentials we are looking for:

  • Diploma or Degree in Disability Studies, or a related social science
  • Demonstrate skills using technology for completing documentation, internet/intranet searches and using email
  • Competent in use of web-based consumer-management system
  • Demonstrate math skills adequate for fund tracking
  • Minimum of two years of experience working with individuals with disabilities or has an equivalent combination of education and directly related experience
  • Well developed and effective communication skills (written and oral)
  • Demonstrated skill in critical thinking
