Edmonton Zone Continuing Care provides services through 5 main program areas: Home and Community Care, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for an Occupational Therapist to join our Home and Community Care (HCC) team to provide professional support services in the homes of clients of all ages & stages of life. You will administer assessments and implement interventions in clinical areas such as functional transfers and mobility, home accessibility, pressure and wound management, cognitive screening and assessment, wheelchair and seating assessment, eating/feeding and swallowing assessments, etc. Restorative and compensatory Interventions may be used to support clients to achieve optimal functional abilities and independence related to their cognitive, affective, spiritual and physical status. These interventions include, but are not limited to, formulating appropriate equipment recommendations and environmental adaptations to improve home accessibility and safety, as well as assisting clients to access relevant community resources to promote health and independence. By implementing OT models and theories, such as the Canadian Occupational Therapy Inter-relational Practice Process (COTIPP) framework and the Person-Environment-Occupation (PEO) model, the HCC OT collaborates with clients and families in determining, implementing and evaluating rehabilitation. Additional models and frameworks utilized in HCC c include components of behavioral activation, ACT (acceptance and commitment therapy), Motivational Interviewing and integration of education related to Chronic Disease Management.

Description:

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

Required Qualifications:

Completion of bachelor's degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT).

Additional Required Qualifications:

Master's Degree in Occupational Therapy. New graduates are welcome to apply as Home and Community Care has a 12-week extensive new hire orientation program. Basic Cardiac Life Support- Healthcare Provider (BCLS HCP). Ability to perform all duties in accordance with related legislation and regulatory bodies. Proven ability to provide exceptional clinical care within a continually changing healthcare environment. Communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment. The successful candidate will work independently in clients' homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Outlook and the Internet.

Preferred Qualifications:

Previous Continuing Care experience. Palliative care experience. AADL authorizer. Experience with Connect Care and the RAI HC assessment tool. Excellent interpersonal and communication skills. Ability to work collaboratively within a team, while also being self-directed. Excellent organizational and time management skills.
