POSITION SUMMARY

A Service Coordinator is an experienced disability worker who gives direct support to individuals with disabilities and takes part in assessment, person-centered planning, and carrying out of plans. It may involve more complicated and/or specialized individual situations or environments where they can use their experience to determine and apply best practice based on the characteristics of the individual and situation, rather than in a routine way. This complexity typically requires more specialized education/training and at least several years of experience covering a broader range of services and supports.

RESPONSIBILITIES

  • Help the individual and their circle of support develop, and document individualized, person-centered plans to guide services related to individual development, community participation, behavior management and/or personal care.
  • Help carry out support plans in more complex situations, without having the responsibility and power of a supervisor, and make communication easy within the group.
  • Help and encourage individuals to advocate on their own behalf, when required to have their needs met; and, when appropriate, support the individual's desires, goals and life choices by interacting and advocating with the individual's social network.
  • Encourage individuals to use their rights and take responsibility to make life choices.
  • Find and develop recreational, volunteer, employment, and other social interaction opportunities for individuals, to create a wider range of options and natural supports within the community.
  • Be a social and/or recreational role model and coach to help the individual participate and contribute effectively within the community.
  • Take personal responsibility for learning about people using the service and the way autism affects them; for listening to each individual; and for reflecting on own practice to continuously develop your own knowledge and skills.
  • Communicate in a professional, timely, accurate and respectful way with individuals, families, guardians, staff, and community members.
  • Document goals/daily log notes
  • Work effectively in a team-oriented environment

PREFERRED QUALIFICATIONS AND SKILLS

  • Valid Class 5 Driver's License, 2M Liability Insurance, Driver's Abstract (required every 3 years) and use of a personal vehicle may be required for this position.
  • A positive attitude and belief that persons with disabilities have the right and responsibility to define and pursue their life choices and pursue active participation in the community.
  • Demonstrated ability to work effectively in a team.
  • Basic computer literacy and well-developed writing/reporting skills that meet the requirements to support service delivery, basic Internet search skills, and the use of basic data reporting and analysis tools.
  • Well-developed observation and interpretive skills to support judgments about what actions will resolve issues; this includes the ability to identify observable changes in behavior and attitude, knowing when it is appropriate to ask for help from a more experienced person.
  • Effective communication skills.
  • Well-developed coaching and mentoring skills, as well as developing supervisory or coordinating skills.
  • Effective time-management and priority-setting skills, and the ability to balance competing demands/priorities and deal with competing opinions. This includes knowing when it is appropriate to ask for help or refer someone to more experienced staff.
  • Relevant knowledge and the ability to model and support the development of basic living, personal and social skills.
  • Knowledge of the broader range of community resources available to support the individual's participation and inclusion, as well as how and when to call on them. This includes the ability to interact effectively with others in the individual's social network.
  • Flexibility to work in physically challenging situations, or a change in shift if required by the specific situation or to fit the needs of an individual's day.
  • Has the skills and abilities to provide job site and volunteer coaching

HOURS OF WORK: Mondy - Friday 8:30am - 4:00 pm (37.5 hr/wk.)
