HHS at Home Navigator

The HHS at Home Navigator will support a unique model of service delivery centred on patient needs and facilitates transition to the most appropriate services that meet the individual and functional goals of the patient. The HHS at Home Navigator works in collaboration with the health care teams across the hospital, the service provider organization (home care delivery team) and other community partners to ensure patients can smoothly transition home with supports in a safe and timely manner. The Navigator provides education and support to HHS teams, patients and their families about the program and works to creatively address barriers to patient flow. The HHS at Home Navigator also takes the lead on evaluating and improving the program to ensure optimal performance and outcomes. The Navigator role will be a multi-site role, providing coverage for the Hamilton General Hospital site and the Juravinski Hospital site.
HHS at Home is an innovative program, part of a provincially standardized network of programs designed to facilitate smooth transition of patients from the hospital setting to their homes (or other treatment locations) to focus on optimization, restorative, reactivation, or rehabilitative care needs. The HHS at Home Program partners with service provider organizations (SPOs), community support services and other health system resources to ensure that patients
receive high quality, efficient, wraparound care and supports in the community setting. Through this work, HHS at Home seeks to enhance patient and caregiver experiences, improve system efficiency and address the growing demands on the healthcare system.
