Job Description

As a Case Manager working in High Intensity Integrated Care Program, you will deliver and advocate for high-quality, client-centered care for patients on your caseload to ensure they can remain safely at home. Working within the defined scope of practice as a Registered Practical Nurse (RPN) or Registered Nurse (RN), your responsibilities include, but are not limited to:

  • Conducting comprehensive in-home client assessments
  • Developing, evaluating, and updating individualized care plans
  • Case management including referral to allied health and support services
  • Providing client education and promoting health and wellness
  • Responding to urgent patient needs to ensure patient outcomes are maximized
  • Monitors, educates, mentors, coaches and evaluates personal and home support workers within their scope of practice.
  • Participates in organizational quality improvement plans.
  • Addresses and follows up on client and employee concerns in a timely manner.

This role requires exceptional customer service skills and close collaboration with an interdisciplinary team to ensure optimal care for community clients. You will monitor service delivery to maintain the highest standards of care and uphold the principles of quality documentation and health record management at all times.

Qualifications:

Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO)

A university degree or college diploma from a recognized educational institution

Minimum 1 year in a community home health setting essential

Leadership or management experience preferred

Care Coordination experience preferred

Process improvement and policy development highly desirable

Proven track record in progressive leadership responsibility

Experience managing operational metrics and team performance

Highly developed written and verbal communication skills
