Overview

We are seeking a dedicated and skilled Wound Care Nurse to join our healthcare team. The ideal candidate will possess a strong background in nursing with specialized knowledge in wound care management. This role is essential in providing high-quality patient care, ensuring effective treatment plans, and promoting optimal healing outcomes for patients with complex wounds. The Wound Care Nurse will work collaboratively with other healthcare professionals to deliver comprehensive care in various settings, including hospitals, home care, and assisted living facilities.

Duties

  • Assess and evaluate patients' wounds to determine appropriate treatment plans.
  • Administer wound care treatments and procedures according to established protocols.
  • Monitor patients' progress and response to treatment, adjusting care plans as necessary.
  • Educate patients and their families on wound care techniques and prevention strategies.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of services.
  • Document patient assessments, interventions, and outcomes accurately in electronic health records (Epic or Cerner).
  • Provide support for pain management related to wound healing processes.
  • Participate in case management activities to facilitate comprehensive patient care.
  • Maintain up-to-date knowledge of best practices in wound care and attend relevant training sessions.

Skills

  • Proficient in nursing practices with a focus on patient care and wound management.
  • Familiarity with electronic health records systems such as Epic or Cerner is preferred.
  • Experience in acute care settings, including hospital environments and nursing homes.
  • Knowledge of pain management techniques and medication administration protocols.
  • Strong understanding of medical terminology relevant to wound care and nursing practices.
  • Background in pediatrics or experience working with children is a plus.
  • Experience in home care settings, assisted living facilities, or senior care environments is beneficial.
  • Ability to perform phlebotomy procedures when necessary for patient assessments.
  • Excellent communication skills for effective interaction with patients, families, and healthcare teams.
