Your Role as a Nurse Case Manager:
You'll be the key point of contact in coordinating holistic care plans tailored to each client's unique needs, ensuring care is not just effective—but empowering.
Responsibilities:
- Collaborate with clients, families, and healthcare professionals to develop personalized care plans.
- Manage client cases from intake to discharge, including assessments and follow-ups.
- Ensure safe, high-quality, and client-centered care delivery.
- Mentor and support staff, conduct regular performance reviews, and hold team meetings.
- Provide ongoing communication and training to care staff about client needs and changes.
- Support supply management, PPE distribution, and care coordination logistics.
- Maintain accurate and timely documentation for all client-related activities.
What You Bring:
- Experience in community health is an asset
- An active LPN license with BCCNM, with no restrictions
- 1-2 years supervisory skills. This includes having experience with decision making, coaching, and negotiation
- Familiarity with the quality management approach and commitment to customer service.
- Valid driver's license
- Current criminal background check will be requested
