Title: Community Worker
Reports to: Executive Director
Schedule: 35 hours per week, Monday to Friday 9:00 to 5:00
Contactivity Centre is a non-profit community centre which provides programs and services to seniors 60+. Seniors join the Centre for social engagement, learning opportunities, health and wellness programs, day program, support groups, lunches, celebrations, and much more both online and in-person.
The Community Worker is responsible for connecting with seniors in the community, especially vulnerable seniors, and providing information and referral to Contactivity and community resources at large. Contactivity services include Telecheck (daily calls), grocery shopping assistance, medical accompaniment, friendly visiting.
Specific responsibilities:
Outreach & Information
· Implement outreach campaigns to raise awareness about community services, Contactivity programs and activities
· Distribute materials at popular community services (e.g., pharmacies, hair salons, library, apartment buildings, medical clinics, grocery stores, etc.)
· Hold information tables in busy areas in the community
· Collaborate with CIUSSS, HLM, Police, Public Security on initiatives to raise awareness about community services and Contactivity
· Become well known to seniors living in the community
· Provide information and referral to Contactivity and external resources
· Keep current on available resources for older adults
· Schedule information sessions on topics of interest to older adults (e.g., taxes, pensions)
Implementation of Services
· Conduct home visits with seniors in need of services
· Develop individual intervention and follow-up plans
· Assist seniors with referrals and follow-up to insure services are implemented from Contactivity and other resources
· Encourage older adults to participate in activities to stay involved in the community
· Liaise with CIUSSS and other public sector case managers as well as health & social service professionals
· Supervise volunteers assisting with outreach & services
Administration
· Keep all client information and progress notes current on Contactivity's database
· Fulfill grant reporting requirements
· Attend meetings as needed, e.g., training programs, information sessions
· Fulfill all other duties as requested by the Executive Director
Requirements:
Bilingual (English & French)
Excellent communication and organization skills
Self-starter and able to work independently
Genuine interest in well-being of older adults
Minimum 5 years experience working in social services or community services preferably with older adults
DEC in Special Care Counselling or other relevant discipline OR Bachelor's degree in relevant discipline, ex. Social Work, Psychology, Human Relations, Gerontology
Knowledge of Microsoft Office 365 and Salesforce an asset
Familiarity with the health and social service system

Le Centre Contactivity est un centre communautaire sans but lucratif qui offre des programmes et des services aux aînés de 60 ans et plus. Les aînés se joignent au Centre pour des activités sociales, des occasions d'apprentissage, des programmes de santé et bien-être, un programme de jour, des groupes de soutien, des dîners, des célébrations et bien plus encore, tant en ligne qu'en personne.
Le travailleur communautaire est responsable d'établir des liens avec les aînés de la communauté, particulièrement les plus vulnérables, et de fournir de l'information et des références vers les ressources du Centre Contactivity et celles offertes dans la communauté. Les services de Contactivity comprennent Télé-Check (appels quotidiens), aide aux courses d'épicerie, accompagnement médical et visites amicales.
Responsabilités spécifiques :
Sensibilisation et information
- Mettre en œuvre des campagnes de sensibilisation sur les services communautaires, les programmes et les activités du Centre Contactivity
- Distribuer du matériel dans les services communautaires fréquentés (p. ex. pharmacies, salons de coiffure, bibliothèques, immeubles d'habitation, cliniques médicales, épiceries, etc.)
- Tenir des kiosques d'information dans des endroits achalandés de la communauté
- Collaborer avec le CIUSSS, les HLM, la police et la sécurité publique sur des initiatives de sensibilisation aux services communautaires et à Contactivity
- Se faire connaître des aînés vivant dans la communauté
- Fournir de l'information et des références vers Contactivity et d'autres ressources externes
- Maintenir à jour ses connaissances sur les ressources disponibles pour les aînés
- Organiser des séances d'information sur des sujets d'intérêt pour les aînés (p. ex. impôts, pensions)
Mise en œuvre des services
- Eff effectuer des visites à domicile auprès des aînés ayant besoin de services
- Élaborer des plans d'intervention et de suivi individualisés
- Aider les aînés avec les références et le suivi afin d'assurer la mise en œuvre des services de Contactivity et d'autres ressources
- Encourager les aînés à participer aux activités pour demeurer impliqués dans la communauté
- Assurer la liaison avec le CIUSSS et d'autres gestionnaires de cas du secteur public ainsi qu'avec les professionnels de la santé et des services sociaux
- Superviser les bénévoles qui participent aux activités de sensibilisation et aux services
Administration
- Maintenir à jour toutes les informations des clients et les notes de suivi dans la base de données de Contactivity
- Répondre aux exigences des rapports liés aux subventions
- Assister aux réunions au besoin (formations, séances d'information, etc.)
- Remplir toutes autres tâches demandées par la directrice exécutive
Exigences :
- Bilingue (anglais et français)
- Excellentes aptitudes en communication et en organisation
- Autonome et capable de travailler de manière indépendante
- Intérêt sincère pour le bien-être des aînés
- Minimum de 5 ans d'expérience dans les services sociaux ou communautaires, de préférence auprès des aînés
- DEC en techniques d'éducation spécialisée ou dans une discipline connexe OU baccalauréat dans une discipline pertinente (ex. : travail social, psychologie, relations humaines, gérontologie)
- Connaissance de Microsoft Office 365 et de Salesforce (un atout)
- Bonne connaissance du réseau de la santé et des services sociaux
