Outreach Worker

TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking one Outreach Worker to join our team in a full-time contract role until June 30, 2026.

The ideal candidate will hold a diploma or degree in Social Services, Community Development, or a related field, with one to three years of experience providing frontline services and community outreach to newcomers; experience working with seniors is an asset. The successful candidate will have demonstrated experience supporting seniors through recreational and therapeutic programming, outreach in a community service setting, and work with at-risk or frail seniors, including those with cognitive impairment, mental health challenges, and mild to moderate medical issues. A strong understanding of health and wellness concepts as they relate to therapeutic program planning is essential. The candidate should also have experience in building and maintaining as well as experience in crisis intervention and dementia-informed programming. Excellent interpersonal, communication, customer service, organizational, and multitasking skills are required, along with strong computer proficiency in Word, Excel, databases, PowerPoint, or Publisher. Certification in Food Handler's Course, CPR, and First Aid is considered an asset. Availability to work weekends, early mornings, and occasional evenings is required.

DUTIES & RESPONSIBILITIES

  • Encourage and engage seniors to actively connect to resources and programming offered throughout the community.
  • Act as a resource to the community and TNO staff by establishing links with community groups, agencies and Community Ambassadors.
  • Develop individualized outreach plans for each assigned service location in collaboration with other TNO programs or external service providers.
  • Actively conduct outreach to the community by organizing/attending community events, promoting TNO programs, updating internal information boards, and working on TNO's promotional materials for Health services.
  • Collaborate with service partners to conduct program promotion in community social landmarks.
  • Identify community members willing to participate in and/or assist with TNO's community engagement and/or development activities.
  • Participate in internal and external committees.
  • Coordinate activities with other TNO departments and services.
  • Actively participates in program's community development activities including advocacy, outreach and community engagement and empowerment activities.
  • Participates in networks of neighbourhood-based service providers.
  • Nurtures community members for leadership.
  • Participates in and supports local community events.
  • Develop and maintain required program records and submit required weekly and monthly reports as requested by the management of the agency.
  • Participates in networks of neighbourhood-based service providers, and support local community events.
  • Networks with other staff, attend staff meetings & agency events and related activities, staff social events, community events, staff and agency committees.

Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Job Type: Full-time contract position until June 30, 2026.

Vacancy Status: This posting is for an existing vacancy – coverage.

Rate: $ $28.94 per hour.

Schedule: 35 hours per week, including two Saturdays per month, early morning shifts, and some evening hours as required.

Work Location(s): All Greater Toronto Area (GTA) locations, including Thorncliffe Park, Flemingdon Park, Teesdale, Taylor Massey, and St. James Town.
