Job Summary

We are seeking a compassionate and detail-oriented Nursing Coordinator to join our expanding home healthcare organization providing personal support care, nursing services, medical equipment rentals and sales, and home and vehicle modifications across Ontario.

The Nursing Coordinator will oversee nursing scheduling, client coordination, and communication between clinical teams, clients, and administrative staff. This key role supports an organization with a robust field staff team, ensuring safe, efficient, and high-quality delivery of nursing and home care services.

Responsibilities

  • Coordinate and schedule nursing visits for home care clients across designated region(s).
  • Serve as a liaison between nursing staff, clients, physicians, and Ontario Health at Home (OHAH).
  • Ensure nursing services meet Ontario Health and OHAH care standards.
  • Maintain up-to-date client documentation and scheduling systems.
  • Support onboarding, credential tracking, and performance management of nursing staff.
  • Collaborate with operations and PSW coordination teams to ensure seamless service delivery.

Qualifications

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO).
  • Minimum 3–5 years of experience in community or home healthcare coordination (preferred).
  • Strong understanding of Ontario Health and OHAH service delivery models (preferred).
  • Excellent organizational, communication, and multitasking skills.
  • Proficiency with scheduling and healthcare documentation systems.
  • Ensure all nursing services comply with CNO practice standards, Ontario Health, and OHAH contractual requirements.
  • Maintain current records of nursing licenses, certifications, and mandatory training.
  • Support quality assurance programs, audits, and incident investigations.
  • Monitor performance indicators related to client outcomes, service timeliness, and satisfaction.
  • Experience in a Windows-based environment
  • Advanced oral and written profficiency in English
  • Valid drivers license and access to own vehicle
  • Vulnerable Sector Police Record Check (or willingness to obtain)

Job Type: Full-time

Pay: $50,000.00-$60,000.00 per year
