Overview

We are seeking compassionate and dedicated Home Support Workers to provide high-quality care and assistance to individuals in their homes and community settings. This role is vital in supporting clients with daily living activities, health monitoring, and emotional well-being. The ideal candidate will have a strong background in home and community care, along with excellent interpersonal skills to foster a safe and supportive environment for clients of all ages and needs. Prior experience in assisted living, long-term care, or working with individuals with developmental disabilities, dementia, or autism is highly valued. Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Provide medication administration and monitor vital signs as prescribed
  • Support clients with memory care needs, including Alzheimer's and dementia care strategies
  • Prepare nutritious meals tailored to individual dietary requirements
  • Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment
  • Use specialized equipment such as Hoyer lifts or resident lifts to ensure safe transfers
  • Observe patient conditions closely, report changes promptly, and document care activities according to HIPAA regulations
  • Support clients during outings or community activities to promote independence and social engagement
  • Follow individualized care plans developed by healthcare professionals or social workers
  • Maintain compliance with all safety protocols including CPR and first aid procedures

Experience

  • Prior experience in home health, assisted living, nursing homes, hospice care, or working with people with disabilities is preferred
  • Knowledge of patient monitoring, medication administration, and vital signs measurement
  • Experience working with individuals with developmental disabilities, autism spectrum disorder, or dementia is advantageous
  • Familiarity with HIPAA regulations and confidentiality standards in healthcare settings
  • Hands-on experience with patient observation, caregiving routines, and long-term care planning
  • Certifications such as CPR and first aid are highly desirable but can be provided during training if necessary
  • Strong interpersonal skills for working effectively with clients, families, and healthcare teams
