Compensation: $43.97/hr - $51.84/hr plus a comprehensive compensation package that includes time-off entitlements, health benefits, life insurance, and participation in the HOOPP pension plan.

What the Program Entails
The Homelessness and Addiction Recovery Treatment (HART) Hub program provides integrated, person-centered supports to individuals with lived experience of homelessness, substance use, and mental health challenges.
What You'll Do
The Clinical Team Lead is a leadership role responsible for overseeing clinical operations, ensuring clinical excellence, and maintaining high-functioning workflows within the HART Hub's interdisciplinary model. This position provides direct oversight of daily clinical activities, supports decision-making across the care continuum, and ensures consistent, safe, and efficient service delivery in alignment with organizational standards and regulatory requirements.
The Clinical Team Lead will play a key role in operational planning, troubleshooting system challenges, and supporting staff in the execution of complex clinical workflows that connect primary care, stabilization, and housing pathways.

What We're Looking For
◈ Your Education Path
- Registered Nurse or Registered Practical Nurse in good standing.
◈ Life and Work Experience
- Minimum 5 years of clinical experience in community health, primary care, mental health, harm reduction, addictions medicine or emergency/urgent care.
- Demonstrated experience in clinical supervision, workflow oversight, or program leadership.
- Demonstrated experience in clinical supervision, including providing guidance, coaching, and oversight to RNs and other clinical staff.
- Strong knowledge of trauma-informed care, withdrawal management, Opioid Agonist Therapy (OAT), harm reduction, crisis de-escalation, and community health practice.
- Excellent clinical judgement, decision-making, communication, and conflict-resolution skills.
- Proven ability to work effectively in fast-paced, high-acuity, and complex service environments.
- Experience working within shelter services, supportive housing, or integrated care models.
- Experience developing, implementing, or optimizing clinical workflows or protocols.
- Experience with various EMRs including PSS.
- Experience with organizational accreditation.
◈ Required Language
- Fluency in English required. Working knowledge of French or other languages commonly spoken in our community is an asset.

Your Day-to-Day at a glance
The Clinical Team Lead's key responsibilities include:
Clinical Workflow & Operational Oversight
- Oversee and coordinate daily clinical operations, ensuring smooth workflow between the drop-in space, primary care, nursing outreach, and stabilization supports.
- Monitor client flow, wait times, and clinical priorities to ensure efficient and safe service delivery.
- Ensure all clinical assessments, treatment protocols, medication processes, and care plans follow regulatory and organizational standards.
- Anticipate and address operational challenges such as staffing gaps, high client volume, and urgent care needs.
- Lead clinical huddles, briefings, and end-of-shift debriefs to maintain situational awareness and team alignment.
Clinical Leadership & Oversight
- Oversee RNs, Medical Office Assistants, and collaborate with Nurse Practitioners.
- Provide advanced clinical judgement and consultation on complex cases, including crisis stabilization, risk assessments, and care planning.
- Ensure clinical documentation, charting, and incident reporting meet compliance and quality standards.
- Review clinical incidents, support root-cause analysis, and contribute to corrective action planning.
- Lead and implement clinical policies, workflows, and protocols across the HART Hub to support consistency and best practice.
Staff Leadership & Performance Support
- Provide leadership, coaching, and mentorship to RNs and frontline clinical staff.
- Support the Program Manager with performance discussions, workload planning, and competency development.
- Oversee orientation and training for new clinical hires and support ongoing professional development.
- Participate in scheduling decisions, coverage planning, and workflow adjustments in collaboration with management.
Client Support & Safety
- Provide clinical support in high-acuity or crisis situations, including overdose response, de-escalation, and stabilization.
- Promote culturally safe, trauma-informed, harm-reduction–centered care across the team.
- Ensure consistent application of client protocols, restrictions, and safety plans.
