ACCOUNTABILITY OBJECTIVE
Are you passionate about making a difference in the lives of seniors? Our UCP's are critical to our communities by supporting people in doing the things that matter most to them while providing a comfortable, safe and nurturing environment. Under the direction of the Health & Wellness Manager, the UCP delivers medication and treatment as per policy and procedure. They participate in planning the care needs of the residents. Delivers care to residents based on individualized care plan, policies and procedures, established standards of care, and as instructed by the registered nurse and registered practical nurse/ resident services manager, and Health and Wellness Manager. Participates in the development, implementation and evaluation of all nursing activities.

Background/Qualifications

  • Minimum of grade 12 education required
  • Has obtained certificate from an approved PSW program, and an approved UCP pharmacology training.
  • Certified in Emergency First Aid Level 1, and CPR.
  • Previous experience as a Health Care Aide/PSW/RSA working with seniors is considered an asset.
  • Ability to communicate effectively, both written and verbally, in the English language, nurturing excellent human relations with residents, families and other team members.
  • Ability to work with supervision and to meet resident care needs, demonstrating initiative and problem-solving skills.
  • Dependable, responsible and an ability to work well with others in a team environment and independently. Demonstrates a positive attitude necessary to meet the goals and standards of the residence.
  • A resident-oriented individual, whose visions and values align with those of the residence.

Key Responsibilities

  • Participates in and adheres to continuous quality improvement standards.
  • Administers medication and treatments in accordance with current approved standards of care, and UCP policies and procedures.
  • Understands and practices pharmacy UCP policies and procedures.
  • Provides direct resident care, in accordance with current and approved standards of care, policies and procedures for effective resident care, taking into account the physical, spiritual, social and emotional needs of the individual.
  • Supports residents' optimal level of functioning in their activities of daily living, while respecting their rights and choices.
  • Assists in monitoring the outcome of specific interventions regularly and offers suggestions for improvement to reporting supervisor. Provides report to oncoming staff.
  • Reports any deviations in the plan of care and reason for it to the immediate supervisor.
  • Maintains accurate records of observations and procedures performed. Completes all required reports.
  • Complies with infection control protocols as directed, e.g. outbreak management.
  • Assists in maintaining a neat, clean and safe environment for residents and personnel. Recognizes safety hazards and takes appropriate action.
  • Assists with safekeeping of all medication in the residence.
  • Reports all incidents and changes in resident status to Health & Wellness Manager, and subsequent shift as well as completes shift report.
  • Participates as a member of the multi-disciplinary team in the development of, and implementation of, a plan of care for residents that supports independence.
  • Utilizes equipment and supplies effectively and in accordance with policies and procedures, manufacturer's instructions, and health and safety regulations.
  • Assists in monitoring the outcome of specific interventions regularly and offers information or suggestions for improvement.
  • Performs laundry and housekeeping duties as assigned.
  • Ensures residents' living space and personal belongings are kept clean, free from safety hazards, and organized according to residents' personal choice.
  • Observes, reports, and documents changes in the residents' physical and emotional condition in a timely fashion that ensures residents' immediate needs are met.
  • Assists with setting up resident breakfast as required.
  • Serves beverages and resident meals in dining room as required.
  • Clears tables in dining room as requested.
  • Participates in the development of department goals and objectives, promoting team building, and achieving customer satisfaction.
  • Maintains effective communication, both written and verbal with co-workers, residents and guests. Uses the appropriate lines of communication with supervisory staff.
  • Performs other duties as assigned by Health and Wellness Manager or delegate.
  • Complies with all corporate policies and procedures.
  • Completes all required mandatory training/education.
  • Complies with the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Material Information System/Globally Harmonized System. This includes recognizing health and safety hazards, reporting incidents, fulfilling responsibilities under the applicable legislation, as well as participating in in-services and fire drills. Participates in the ongoing Fire Prevention, and Pest Control programs and any other programs as required.
