Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH's larger strategy that introduce innovative and inclusive programs and services and new models of care delivery. Position Summary
The Nurse Practitioner (NP) provides direct, comprehensive care to long-term care (LTC) facility residents with specialized and complex needs, utilizing advanced clinical knowledge and skills. The NP practices to the full scope of their license and works collaboratively within an interdisciplinary team.
Key ResponsibilitiesFormulates differential diagnoses by integrating resident data, evidence-informed practice, and determinants of health in collaboration with physicians, nurses, PSWs, and other team members.
Performs comprehensive or focused physical examinations and identifies normal and abnormal findings relevant to the resident's presentation.
Anticipates and diagnoses emergent, urgent, and life-threatening conditions.
Orders or performs screening and diagnostic investigations, interprets results using clinical reasoning, and ensures appropriate follow-up.
Diagnoses diseases, disorders, injuries, and health conditions while considering the resident's response to illness.
Communicates assessment findings, diagnoses, outcomes, and prognoses to residents, families, or substitute decision-makers (SDMs).
Prescribes pharmacotherapy based on resident health history, diagnosis, life stage, and individual needs.
Applies pharmacological knowledge and evidence-based practice in prescribing, monitoring, and managing medications.
Educates families/SDMs about medication therapy, benefits, side effects, and potential interactions.
Monitors, evaluates, and revises care plans and interventions based on evidence and resident outcomes and goals.
Assesses and manages residents with acute illness, chronic disease, comorbidities, and complex health needs.
Provides relevant diagnostic information and education using appropriate teaching and learning strategies.
Considers resource allocation and cost-effectiveness in clinical decision-making.
Accurately documents clinical data, assessments, diagnoses, care plans, and rationale in a timely manner.
Collaborates with residents and SDMs to develop person-centered care plans.
Refers residents to other healthcare providers when outside NP scope of practice or expertise.
Works interprofessionally to promote resident-centered care at individual, organizational, and systems levels.
Coordinates resident care with external providers, community agencies, and resources.
Promotes resident quality of life, independence, and positive aging.
Practices based on current research, best practice guidelines, and clinical evidence.
Evaluates interventions and adjusts care plans in collaboration with residents, SDMs, and the care team.
Coordinates discharge planning and transitions to and from other healthcare settings.
Provides and advocates for palliative and end-of-life care.
Supports LTC staff by integrating and promoting best practices in care.
Participates in research and quality improvement initiatives.
Provides leadership in clinical care management; acts as a resource, educator, and role model (e.g., during huddles, care conferences, interdisciplinary rounds).
QualificationsBachelor's Degree in Nursing; Master's in Nursing preferred
Completion of the Ontario Primary Health Care Nurse Practitioner Certificate Program
Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
Membership with the Nurse Practitioners' Association of Ontario (NPAO) and Registered Nurses' Association of Ontario (RNAO)
Canadian Gerontological Nursing certification or plans to obtain certification
Minimum 5 years' experience in gerontological nursing or primary health care as a Nurse Practitioner (preferred)
Experience in geriatrics and/or geriatric psychiatry, particularly in managing Behavioural and Psychological Symptoms of Dementia (BPSD) and responsive behaviours (preferred)
Experience working in long-term care settings
Strong knowledge of:
CNO standards
LTC and pharmacy standards
Regulated Health Professions Act
Infection control and risk management practices
Proven leadership abilities and strong communication (oral, written, interpersonal) skills
Demonstrated alignment with core and leadership competencies and behaviors
